血清IL-6、PCT、hs-CRP诊断新生儿细菌性感染肺炎的临床意义

Clinical significance of serum IL-6,PCT and hs-CRP in diagnosis of neonatal bacterial infection pneumonia

目的探讨血清白细胞介素6(IL-6)、血清降钙素原(PCT)、超敏C反应蛋白(hs-CRP)对诊断新生儿细菌性感染肺炎的临床意义。方法选取2018年3月至2020年1月收入我院新生儿科的细菌性感染肺炎83例作为观察组,选取同期体检的健康新生儿79例作为对照组,对两组患儿血清IL-6、PCT、hs-CRP等指标进行检测,对结果进行分析。结果观察组患儿血清IL-6、PCT、hs-CRP水平高于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组血清IL-6、PCT、hs-CRP阳性率比治疗前均降低,差异具有统计学意义(P<0.05);治疗后,观察组IL-6、PCT及hs-CRP指标阳性率比较差异无统计学意义(P>0.05)。PCT的敏感性、特异性、阳性预测率及阴性预测临率高于IL-6和hs-CRP6。结论血清IL-6、PCT、hs-CRP可作为新生儿细菌性感染性肺炎的鉴别指标,其中PCT可作为新生儿细菌性感染性肺炎的首选血清指标。

Objective To explore the clinical significance of serum interleukin 6(IL-6),serum procalcitonin(PCT),and hypersensitive C-reactive protein(hs-CRP)in the diagnosis of neonatal bacterial infection pneumonia.Methods Eighty-three neonates with bacterial infection pneumonia admitted to our neonatology department from March 2018 to January 2020 were enrolled as the observation group, and 79 healthy neonates during the same period were selected as the control group.The PCT,hs-CRP and other indicators were measured to analyze the results.Results The serum IL-6,PCT and hs-CRP levels in the observation group were higher than those in the control group(P<0.05).After treatment, the positive rates of serum IL-6,PCT and hs-CRP in the observation group were lower than before treatment(P<0.05).After treatment, there was no statistically significant difference in the positive rates of IL-6,PCT and hs-CRP indicators in the observation group(P>0.05).The sensitivity, specificity, positive prediction rate and negative prediction rate of PCT were higher than those of IL-6 and hs-CRP6.Conclusion Serum IL-6,PCT and hs-CRP can be used as indicators for the identification of neonatal bacterial infectious pneumonia, which PCT can be used as the prior serum indicator for neonatal bacterial infectious pneumonia.
