腹部超声在成人原发性腹部淋巴管瘤中的诊断价值

The Diagnostic Value of Abdominal Ultrasound in Adult Primary Abdominal Lymphangioma

目的探究腹部超声在成人原发性腹部淋巴管瘤(APAL)中的诊断价值。方法选取2018年1月至2019年12月温县人民医院收治的114例APAL患者的病历资料进行回顾性分析,男性55例,女性59例,采用超声系统进行检查,观察患者超声特征,记录诊断信息。结果 114例APAL患者,病灶长径1.5~24.8cm,平均(11.2±7.3)cm,短径1.4~12.3cm,平均(5.8±3.1)cm,短长径比值为0.18~0.95,平均(0.59±0.48)。彩色多普勒显示19例分隔上可显示血流,23例病灶内低回声区或高回声区见血流信号,72例病灶内未见血流信号。83.1%患者病灶边界清晰,术前超声定位诊断准确率为22.9%,其余病灶错误定位于胰腺、卵巢、肝脏或未明确定位,11例(9.3%)APAL超声定性诊断正确,其余误诊为囊肿、腹腔积液、囊腺瘤等或无定性诊断。结论 APAL的超声表现多样,典型表现为多房囊性占位,超声对成人APAL的定位及定性诊断具有一定价值。

Objective Exploration of the diagnostic value of abdominal ultrasound in adult primary abdominal lymphangioma.Methods The medical records of 114 APAL patients admitted to Wenxian People′s Hospital from January 2018 to December 2019 were selected for retrospective analysis.There were 55 males and 59 females.The ultrasound system was used for examination and the ultrasound characteristics of the patients were observed.,Record diagnostic information.Results In 114 patients with APAL,the length of the lesion was 1.5~24.8 cm, with an average of(11.2±7.3)cm, and its short diameter was 1.4 to 12.3 cm, with an average of(5.8±3.1)cm.The ratio of short to long diameter was 0.18 to 0.95,with an average of(0.59±0.48).Color Doppler showed that 19 cases showed blood flow on the partition, 23 cases showed blood flow signal in hypoechoic area or hyperechoic area, and 72 cases showed no blood flow signal.83.1% of the patients had clear lesion boundaries, and the accuracy of preoperative ultrasound positioning was 22.9%.The remaining lesions were incorrectly located in the pancreas, ovary, liver or not clearly located.11 cases(9.3%)were correctly diagnosed by APAL ultrasound, and the rest were misdiagnosed as cysts.Abdominal effusion, cystadenoma, etc.or no qualitative diagnosis.Conclusion The ultrasound manifestations of APAL are diverse.The typical manifestation is multilocular cystic space.Ultrasound has certain value in the localization and qualitative diagnosis of adult primary abdominal lymphangioma.
