探究消化内镜治疗非静脉曲张性上消化道出血后再出血的危险因素

To explore the risk factors of recurrent bleeding after non-varicose upper gastrointestinal bleeding treated with digestive endoscopy

目的探究消化内镜治疗肺静脉曲张性上消化道出血后再出血的危险因素。方法选取2017年4月-2019年5月期间我院收治的采用消化内镜治疗非静脉曲张性上消化道出血患者440例,根据术后情况将其分为出血组(120例)与未出血组(320例),比较两组患者的检查结果,找出使用消化内镜治疗后再出血的危险因素。结果鼻胃管负压引流阳性发现、低白蛋白血症、慢性肾脏病、胃体部出血、内镜下高危出血征象、心跳过速、恶性肿瘤是造成术后再出血的主要危险因素(P<0.05)。结论慢性肾脏疾病作为非静脉曲张性消化道出血患者再出血的独立危险因素,是临床治疗中需要重点预防及控制的重点危险因素,可以使用消化内镜结合住院跟踪治疗的方式,降低再出血发生率。

Objective To explore the risk factors of recurrent hemorrhage after upper gastrointestinal hemorrhage caused by pulmonary varicose veins treated with digestive endoscopy.Methods Select in April 2017-2019 in our hospital during the period of may adopt digestive endoscopic treatment of 440 cases of patients with upper gastrointestinal bleeding varices sex, according to the postoperative situation will be divided into bleeding group(120 cases)and not bleeding group(320 cases),comparison of two groups of patients with test results, find out the use of digestive endoscopy to bleeding risk factors after treatment.Results Negative pressure drainage of nasogastric tube positive findings, low albuminemia, chronic kidney disease, gastric bleeding, endoscopic signs of high-risk bleeding, tachycardia, malignant tumor are the main risk factors for postoperative bleeding(P<0.05).Conclusion Chronic kidney disease, as an independent risk factor for recurrent bleeding in patients with non-varicose gastrointestinal bleeding, is a key risk factor that needs to be focused on prevention and control in clinical treatment.Digestive endoscopy combined with follow-up treatment in hospital can be used to reduce the incidence of recurrent bleeding.
