摘要
目的:分析对四肢骨关节隐匿性骨折患者采取CT与MRI扫描三维重建诊断的临床价值。方法:选取我院2020年3月—2021年2月收治的48例疑似四肢骨关节隐匿性骨折患者,实施CT与MRI扫描三维重建诊断,将手术病理检查作为金标准,分析其诊断准确度。结果:经检测,金标准检查结果为43例患者为四肢骨关节隐匿性骨折,剩余5例患者为其他疾病。MRI扫描三维重建检查为四肢骨关节隐匿性骨折患者41例,剩余7例患者为其他疾病。CT诊断为四肢骨关节隐匿性骨折患者36例,剩余12例患者为其他疾病。MR I扫描三维重建检查诊断后,特异度为80.00%(4/5)、灵敏度为93.02%(40/43)、准确度为91.67%(44/48),CT诊断后,特异度为20.00%(1/5)、灵敏度为74.42%(32/43)、准确度为68.75%(33/48),对比CT诊断和MRI扫描三维重建检查结果,MRI扫描三维重建检查诊断的灵敏度及准确度均显著高于CT诊断(P<0.05);MRI扫描三维重建检查诊断的特异度高于CT诊断但差异无统计学意义(P>0.05)。结论:四肢骨关节隐匿性骨折患者采取MRI扫描三维重建诊断,临床价值高。
Objective To analyze the clinical value of CT and MRI three-dimensional reconstruction in the diagnosis of occult fractures of limbs and joints.Methods A total of 48 patients with suspected occult fractures of extremities and joints were selected in our hospital from March 2020 to February 2021.CT and MRI scans were used for three-dimensional reconstruction diagnosis.The surgical pathological examination was used as the gold standard to analyze the accuracy of diagnosis.Results After testing,the gold standard examination showed that 43 patients had occult fractures of the limbs and joints,and the remaining 5 patients had other diseases.Three-dimensional reconstruction of MRI scan showed 41 patients with occult fractures of limbs and joints,and the remaining 7 patients had other diseases.CT diagnosed 36 patients with occult fractures of the limbs and joints,and the remaining 12 patients had other diseases.After MRI scan three-dimensional reconstruction check diagnosis,the specificity is 80.00%(4/5),the sensitivity is 93.02%(40/43),the accuracy is 91.67%(44/48),after CT diagnosis,the specificity is 20.00%(1/5),the sensitivity is 74.42%(32/43),and the accuracy is 68.75%(33/48).Comparing the results of CT diagnosis and 3D reconstruction of MRI scan,the sensitivity and accuracy of MRI three-dimensional reconstruction were significantly higher than that of CT the sensitivity and accuracy of MRI three-dimensional reconstruction were significantly higher than that of CT(P<0.05),and the specificity of MRI scan three-dimensional reconstruction examination was significantly higher than that of CT diagnosis,but it was not statistically significant(P>0.05).Conclusion Patients with occult fractures of the limbs and joints are diagnosed with MRI scan and three-dimensional reconstruction,which has high clinical value.
