早期肠内营养与肠外营养对应激性溃疡患者CRP水平的影响研究

Effect of Early Enteral Nutrition and Parenteral Nutrition on CRP Level in Patients with Stress Ulcer

目的研究早期肠内营养与肠外营养对应激性溃疡患者C反应蛋白(CRP)水平的影响。方法选取2020年4月-2021年8月在我院治疗的60例应激性溃疡患者临床资料为研究对象,采用随机数字表法分为对照组和观察组,各30例。对照组采用肠外营养进行治疗,观察组采用早期肠内营养进行治疗,比较两组治疗总有效率、血清CRP水平、胃液pH、APACHEⅡ评分、医院感染发生率、溃疡指数。结果观察组治疗总有效率为93.33%,高于对照组的80.00%(P<0.05);治疗后,两组血清CRP水平均低于治疗前,胃液pH均高于治疗前,且观察组CRP低于对照组,胃液pH高于对照组(P<0.05);治疗后,两组APACHEⅡ评分均低于治疗前,且观察组低于对照组(P<0.05);观察组医院感染总发生率为13.33%,低于对照组的33.33%(P<0.05);治疗后,两组溃疡指数均低于治疗前,且观察组低于对照组(P<0.05)。结论早期肠内营养与肠外营养对应激性溃疡患者CRP水平均具有一定影响,但是早期肠内营养在提高治疗总有效率、改善胃液pH值、降低医院感染发生率、降低APACHEⅡ评分以及溃疡指数方面具有显著的优势。

Objective To study the effect of early enteral nutrition and parenteral nutrition on CRP level in patients with stress ulcer.Methods The clinical data of 60 patients with stress ulcer treated in our hospital from April 2020 to August 2021 were selected as the research objects.They were divided into control group and observation group by random number table method,with 30 cases in each group.The control group was treated with parenteral nutrition,and the observation group was treated with early enteral nutrition.The total effective rate,serum CRP level,gastric juice pH,APACHEⅡscore,hospital infection rate and ulcer index were compared between the two groups.Results The total effective rate of treatment in the observation group was 93.33%,which was higher than 80.00%in the control group(P<0.05).After treatment,the levels of serum CRP in the two groups were lower than those before treatment,the pH of gastric juice was higher than that before treatment,and the CRP in the observation group was lower than that in the control group,the pH of gastric juice was higher than that in the control group(P<0.05).After treatment,the APACHEⅡscores of the two groups were lower than those before treatment,and the observation group was lower than the control group(P<0.05).The total incidence of nosocomial infection in the observation group was 13.33%,which was lower than 33.33%in the control group(P<0.05).After treatment,the ulcer index of the two groups was lower than that before treatment,and the observation group was lower than the control group(P<0.05).Conclusion Early enteral nutrition and parenteral nutrition have a certain effect on CRP level in patients with stress ulcer,but early enteral nutrition has significant advantages in improving the total effective rate of treatment,improving the pH value of gastric juice,reducing the incidence of nosocomial infection,reducing APACHEⅡscore and ulcer index.
