探讨PCOS不孕症患者应用不同促排卵方案联合宫腔内人工授精治疗的临床效果及妊娠结局

Objective to Investigate the clinical effect and Pregnancy outcome of different Ovulation induction programs Combined with intrauterine Insemination in patients with PCOS Infertility

目的观察多囊卵巢综合征(PCOS)不孕症患者采取不同促排卵方案联合宫腔内人工授精治疗的临床效果及妊娠结局。方法选取60例接受宫腔内人工授精的PCOS不孕症患者,采用系统抽样法分为对照组与研究组,每组30例。对照组采用来曲瞠治疗,研究组采用来曲哩联合低剂量注射用尿促性素(HMG)治疗,比较两组成熟卵泡数、HCG注射日子宫内膜厚度、排卵率、流产率及临床妊娠率。结果两组成熟卵泡数比较,差异无统计学意义(P>0.05),研究组HCG注射日子宫内膜厚度薄于对照组,排卵率明显高于对照组,差异具有统计学意义(P<0.05)。研究组流产率、临床妊娠率分别为0、26.67%,对照组流产率、临床妊娠率分别为13.33%、6.67%,研究组流产率明显低于对照组,临床妊娠率高于对照组,差异具有统计学意义(P<0.05)。结论PCOS不孕症患者宫腔内人工授精过程中,来曲哩联合低剂量HMG治疗具有较高的应用价值,该方法有助于提高妊娠率,且流产率低,值得临床进一步推广。

Objective To observe the clinical effect and pregnancy outcome of infertility patients with polycystic ovary syndrome(PCOS)treated with different ovulation induction schemes combined with intrauterine artificial insemination.Methods 60 PCOS infertile patients who received intrauterine artificial insemination were selected and divided into control group and study group by systematic sampling method,with 30 cases in each group.The control group was treated with Letrozole,and the study group was treated with letrozole combined with low-dose urinary gonadotropin(HMG).The number of mature follicles,intrauterine thickness,ovulation rate,abortion rate and clinical pregnancy rate were compared between the two groups.Results There was no significant difference in the number of mature follicles between the two groups(P>0.05).The intrauterine membrane thickness in the study group was thinner than that in the control group on the day of HCG injection,and the ovulation rate was significantly higher than that in the control group,with statistical significance(P<0.05).The abortion rate and clinical pregnancy rate in the study group were 0 and 26.67%,respectively,while the abortion rate and clinical pregnancy rate in the control group were 13.33%and 6.67%,respectively.The abortion rate in the study group was significantly lower than that in the control group,and the clinical pregnancy rate was higher than that in the control group,with statistical significance(P<0.05).Conclusion Letrozole combined with low-dose HMG is of high application value in intrauterine artificial insemination of PCOS infertile patients.This method can improve the pregnancy rate,and abortion is low,which is worthy of further clinical promotion.
