摘要
目的运用磁共振扩散张量成像(DTI)技术研究阿尔茨海默病(AD)患者脑白质微观结构的变化特点,探讨DTI对AD的诊断价值。方法采用Signa Hde GE1.5T磁共振成像仪,收集老年科医师诊断的20例AD患者(AD组)和20例性别、年龄和受教育程度相匹配的老年健康志愿者(对照组),所有受试者均行DTI扫描,分别测量两组受试者不同脑区的FA值、ADC值,比较两组间的脑白质微观结构DTI参数变化差异。结果与正常对照组对比,AD组患者双侧下纵束、双侧下额枕束、双侧内囊前肢、双侧内囊后肢、双侧后扣带回、胼胝体膝部及压部FA值均减低,左侧钩束及下纵束、左侧内囊后肢、胼胝体膝部及压部ADC值增加,差异具有统计学意义(P均<0.05)。结论DTI技术的FA值、ADC值能定量反映AD患者不同脑区的微观结构异常,可以评估AD患者脑白质完整性,为AD提供更多辅助诊断信息。
Objective To study the microstructure changes of white matter in Alzheimer′s disease(AD)patients and to explore the diagnostic value of DTI in AD.Methods 20 cases of AD patients diagnosed by geriatrician(AD group)and 20 elderly healthy volunteers(control group)matched by gender,age and level of educationwere collected,all subjects underwent DTI scanning by using Signa Hde GE1.5T magnetic resonance imaging(MRI),FA and ADC values in different brain regions of the two groups were respectively measured to compare the DTI parameters differences in the microstructure changes of white matter between the two groups.Results Compared with the normal control group,FA values of bilateral inferior longitudinal fasciculus,bilateral inferior fronto-occipital fascicles,bilateral anterior limbs of internal capsules,bilateral posterior limbs of internal capsules,bilateral posterior cingulate gyrus,knee of corpus callosum and compression of corpus callosum of AD group were decreased,the ADC values of left inferior longitudinal fasciculus,left uncinate fasciculus,left posterior limb of internal capsule,knee of corpus callosum and compression of corpus callosum increased significantly(all P<0.05).Conclusion The FA and ADC values of DTI can quantitatively reflect the microstructural abnormalities in different brain regions of AD patients,and can evaluate the integrity of the white matter of AD patients,which can provide more auxiliary diagnostic information for AD.
