HPV分型与液基细胞学联合检测在宫颈癌及癌前病变筛查中的效能

Efficacy of combined detection of HPV typing and liquid-based cytology in screening for cervical cancer and precancerous lesions

目的探讨人乳头状瘤病毒(HPV)分型与液基细胞学联合检测在宫颈癌及癌前病变筛查中的效能。方法选取2017年1月至2019年12月收治的接受HPV分型与液基细胞学筛查宫颈病变的女性60例进行观察,所有患者均经HPV分型、液基细胞学检测、病理学检查,以病理学结果为金标准,比较分析HPV分型、液基细胞学单独与联合检测在宫颈癌及癌前病变筛查中的效能。结果60例患者经病理检查显示,宫颈癌8例,CIN 1级13例,CIN 2级14例,CIN 3级13例,正常12例;HPV分型检测示真阳性40例,真阴性11例;液基细胞学检测示真阳性30例,真阴性11例;HPV分型联合液基细胞学检测示真阳性47例,真阴性12例。HPV分型联合液基细胞学检测的灵敏度、准确度、阴性预测值明显高于单独HPV分型、液基细胞学检测(P<0.05)。宫颈癌及癌前病变患者的HPV分型阳性率及液基细胞学检测阳性率均显著高于正常组(P<0.05)。结论临床在宫颈癌及癌前病变筛查中采用HPV分型与液基细胞学联合检测有助于提高诊断效能,宫颈癌及癌前病变患者的HPV分型阳性率及液基细胞学检测阳性率均明显高于正常者。

Objective To explore the efficacy of combined detection of human papilloma virus(HPV)typing and liquid-based cytology in screening for cervical cancer and precancerous lesions.Methods Sixty women undergoing HPV typing and liquid-based cytology screening for cervical lesions from January 2017 to December 2019 were selected for observation.The women were diagnosed by HPV typing,liquid-based cytology and pathological examination.With the pathological results as the gold standard,the efficacy of HPV typing,liquid-based cytology alone and combined detection in the screening of cervical cancer and precancerous lesions were compared and analyzed.Results Pathological examination of 60 patients showed 8 cases of cervical cancer,13 cases of CIN 1 grade,14 cases of CIN 2 grade,13 cases of CIN 3 grade,and 12 normal cases;HPV typing showed 40 cases of true positive and 11 cases of true negative;liquid-based cytology showed 30 cases of frue positive,and 11 cases of true negative;combined detection of HPV typing and liquid-based cytology showed 47 cases of true positive,and 12 cases of true negative.The sensitivity,accuracy and negative predictive value of HPV typing combined with liquid-based cytology were significantly higher than those of HPV typing and liquid-based cytology alone(P<0.05).The positive rates of HPV typing and liquid-based cytology in patients with cervical cancer and precancerous lesions were significantly higher than those in the normal group(P<0.05).Conclusion The clinical use of HPV typing and liquid-based cytology in the screening of cervical cancer and precancerous lesions can help improve the diagnostic efficiency.The positive rates of HPV typing and liquid-based cytology in patients with cervical cancer and precancerous lesions were significantly higher than those in normal subjects.
