摘要
目的:分析老年抑郁症患者抗抑郁药物门诊处方用药情况,为临床合理用药提供参考。方法:通过HIS系统查询我院2020年精神专科门诊老年抑郁症患者抗抑郁药的使用情况,,对抗抑郁药的品种、用药频度、合并疾病、合并用药、药物间相互作用(DDIs)等进行统计分析。结果:966例老年抑郁症患者中抗抑郁药物使用频次最高的是文拉法辛(27.9%),其次是舍曲林(20.79%),阿米替林使用频次最低(1.69%)。老年抑郁症患者最常见的合并疾病为高血压(27.0%)、而女性患者合并神经功能紊乱更为突出(5.6%);应用1种精神类药物治疗的603例(62.42%),联合2种精神类药物治疗的患者362例(37.57%);老年抑郁症患者药物处方中潜在DDIs的总体发生率为16.1%。结论:门诊老年抑郁症患者处方用药基本合理,仍然存在一些不合理处方,主要是伴有慢性基础疾病的患者用药,应避免发生药物间相互作用,注意合理用药。
Objective:To analyze the rational use of antidepressants in outpatient prescriptions of senile depression patients,and to provide reference for clinical rational use of antidepressants.Methods:Through HIS system,to inquire the use of antidepressants in outpatient department of our hospital in 2020,to select 966 prescriptions of elderly patients with depression in psychiatric outpatient department of our hospital,and to statistically analyze the varieties of antidepressants,medication frequency,combined diseases,combined drugs,drugs-drug interactions(DDIs).Results:Among 966 patients with senile depression,venlafaxine(27.9%)was the most frequently used.Sertraline(20.79%)was the second,and Amitriptyline(1.69%)was the least frequently used.The most common comorbid diseases of senile depression patients were hypertension(27.0%),and the neurologic disorders of female patients were more prominent(5.6%).Among 966 patients with senile depression,603 cases(62.42%)were treated with one kind of psychoactive drugs,and 362 cases(37.57%)were treated with two kinds of psychoactive drugs.The overall incidence of potential DDIs was 16.1%.Conclusion:The outpatient prescriptions were basically reasonable for senile depression patients,but there were still some unreasonable prescriptions,especially for patients with chronic underlying diseases.In order to avoid drug interaction,we should pay attention to the safe and reasonable use of drugs.
