真实世界中参芎葡萄糖注射液治疗老年人群联合用药特征分析

Clinical Application Characteristics and Medication Analysis of Shenxiong Glucose Injection in Elderly Population Based on Real World

目的:本研究深入分析真实世界中使用参芎葡萄糖注射液老年人群的用药特征,为参芎葡萄糖注射液的合理用药与进一步研究提供参考借鉴。方法:基于真实世界电子医疗病历系统(Hospital Information System, HIS),提取使用参芎葡萄糖注射液的65岁以上老年人群诊疗数据,对患者基本信息、诊断信息等进行描述性分析,并通过Apriori算法关联规则的分析方法对合并用药深入挖掘,探索其临床使用规律。结果:全国19家三甲医院共2 466例使用参芎葡萄糖注射液的老年患者被纳入研究,男性多于女性,年龄中位数为74岁,单次剂量100 mL~200 mL者为主,用药疗程以8 d~14 d者居多;真实世界中使用参芎葡萄糖注射液的老年患者,常联合使用阿司匹林、阿托伐他汀、氨溴索、呋塞米、泮托拉唑、疏血通注射液及痰热清注射液等中西药物。结论:本研究基于真实世界数据,揭示了使用参芎葡萄糖注射液老年人群在当前医疗环境中真实的临床用药现状,基本情况符合相关指南,探索发现了抗血小板聚集、降血脂与降血压、中医特色的活血化瘀与清热化痰等四种临床常用治疗方案,在促进参芎葡萄糖注射液的临床合理用药等方面有着重要的参考价值。

Objective:To analyze the characteristics of Shenxiong glucose injection in the real world and provide the reference for the rational use of Shenxiong glucose injection. Methods: Based on the real-world electronic medical case system(Hospital Information System, HIS), the diagnosis and treatment data of the people aged over 65 were screened, who were given Shenxiong glucose injection. The basic information and diagnosis information of the patients were analyzed descriptively, and the Apriori algorithm association rules were used to dig deeply into the clinical use rules of combination drugs. Results: A total of 2466 elderly patients treated with Shenxiong glucose injection were enrolled to the study in 19 third-class hospitals in China. The median age was 74 years old, patients were given a single dose of 100 mL-200 mL, the male patients were more than female patients, and the duration of treatment was mostly 8-14 days. In real world, the elderly patients often used Shenxiong glucose injection combined with aspirin, atorvastatin, ambroxol, furosemide, pantoprazole, Shuxuetong injection or Tanreqing injection. Conclusion: Based on the real world data, this study reveals the actual clinical use of Shenxiong glucose injection in the elderly people, the basic situation is in line with relevant guidelines. Four common clinical treatment programs were found: anti-platelet aggregation, lowering blood lipid and blood pressure, Huoxuehuayu and Qingrehuatan in traditional Chinese medicine treatment, which showed important reference value in promoting clinical rational use of Shenxiong glucose injection.
