倒班作业对石油化工企业男性工人总胆固醇与三酰甘油水平的影响

Effects of shift work on total cholesterol and triglyceride levels in male workers in petrochemical enterprises

摘要 目的了解倒班作业对石油化工企业男性工人总胆固醇与三酰甘油水平的影响和倒班作业与工人高脂血症患病率的关系,以便更好地为倒班作业人群提供全面的健康指导和管理。方法采用整群抽样的方法,2018年选取广东省某石油化工企业1 047名倒班作业的男性工人为调查对象。采用自行设计的个人健康状况调查表对工人的基本资料、心血管疾病家族史、工作类型、倒班年限、吸烟史和饮酒史等进行调查。根据倒班年限的P20间距把倒班作业男性工人分为5组,比较5组的男性工人三酰甘油(triacylglycerol,TG)、总胆固醇(total cholesterol,TC)、高密度脂蛋白胆固醇(high density lipoprotein chdesterol,HDL-C)水平和高脂血症患病率的差异。应用多元线性回归模型分析倒班年限与TC和TG水平的关系和利用二分类多因素的logistic回归模型探讨不同倒班年限组人群与高脂血症患病率的关系。结果倒班年限不同的5组中E组人群在血压异常、高血糖、低HDL-C和吸烟的人数比例最高,分别是54.1%、2.6%、16.5%和38.1%(P<0.05);体质指数(body mass index,BMI)、TC和TG的水平最高,分别是24.8 kg/m2、5.5 mmol/L和1.6 mmol/L(P<0.05)。应用多元线性回归模型发现随着倒班年限的增加,TC和TG的水平均升高(P<0.05)。二分类多因素logistic回归模型发现C、D、E组患高脂血症的风险均高于A组(P<0.05),即倒班年限>5年人群患高脂血症的风险均高于倒班年限≤4年的人群。D组人群患高脂血症的风险最高,为A组的2.345倍。结论倒班年限的延长可使男性工人TC和TG水平升高,倒班作业增加了患高脂血症的风险,倒班作业与高脂血症的患病率之间存在相关性。 Objective To understand the impact of shifting operations on total cholesterol and triglyceride levels in male workers in petrochemical enterprises and the relationship between shift-work and the prevalence of hyperlipidemia in male workers in petrochemical enterprises,provide comprehensive health guidance and management for shift workers. Methods A cluster sampling was used to select 1 047 male workers for shift work in a petrochemical industry in Guangdong province in 2018. The self-designed personal health survey questionnaire was used to investigate basic data,family history of cardiovascular disease,job type,years of shift work,smoking history,and drinking history. Male workers in shifting work were divided into five groups according to the P20 spacing of shift years,and the differences in the levels of triglycerides(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C)and the prevalence of hyperlipidemia among male workers in the five groups were compared. The multivariate linear regression model was used to analyze the relationship between shift years and levels of TC and TG. Binary multivariate logistic regression model was used to investigate the relationship between the shift years and the prevalence of hyperlipidemia.Results Among the five groups with different shift years,the proportions of people with abnormal blood pressure,hyperglycemia,low HDL-C and smoking werethe highest in group E,which was 54.1%,2.6%,16.5% and 38.1%,respectively(P<0.05),and the levels of body mass index(BMI),TC and TG were the highest,which was 24.8 kg/m2,5.5 mmol/Land 1.6 mmol/L,respectively(P<0.05).The multivariate linear regression model showed that the levels of TC and TG increased with the increase of shift years(P<0.05).Binary multivariate logistic regression model found that the risk of hyperlipidemia in group C,D and E was higher than that in group A(P<0.05),that is,the population with shift work more than 5 years was higher than those with shift work less than 4 years.Moreover,the group D was found to have the highest risk of hyperlipidemia,which was 2.345 times that of group A.Conclusion The lengthening of shifting work years can increase the levels of TC and TG in male workers. The shift work increases the risk of hyperlipidemia,and there is a correlation between the shift work and the prevalence of hyperlipidemia.
