分阶段宣教持续质量改进对心脏大血管外科术后患者抗凝治疗知信行的影响

The effect of continuous quality improvement in staged missionary education on the knowledge,attitude,belief and practice of anticoagulant patients after cardiac and vascular surgery

目的:探讨分阶段宣教持续质量改进项目活动对心脏大血管外科术后患者抗凝治疗知信行的影响。方法:选取本院2018年7月至2019年6月心脏大血管外科术后患者300例,将2018年7月至2018年12月140例术后接受抗凝治疗常规健康宣教干预的患者作为对照组,将2019年1月至2019年6月接受分阶段宣教持续质量改进项目活动干预的160例患者作为观察组。跟踪随访6个月,对比两组患者华法林药物抗凝治疗知信行情况问卷调查及专项活动满意度评价的统计学比较和分析结果。结果:患者出院后随访半年发现,两组患者抗凝药物治疗知信行调查结果及专项活动满意度评价有明显差异,观察组患者对华法林药物知识的知晓率、服用药物的信念和态度、服药相关及门诊复查依从性及健康宣教满意度均明显优于对照组,差异性明显,具统计学意义(P<0.05)。结论:分阶段宣教持续质量改进项目活动对心脏大血管外科术后患者服用抗凝治疗药物的知信行影响较大,患者对抗凝治疗药物的知晓率明显提高,术后抗凝并发症明显减少,保证了患者手术治疗安全性,患者满意度明显提高。

Objective:To explore the impact of staged missionary continuous quality improvement project activities on the knowledge,attitude,belief and practice of anticoagulant patients after cardiac and vascular surgery.Methods:A total of 300 patients who underwent cardiac and vascular surgery from July 2018 to June 2019 in our hospital were selected,and 140 patients who underwent anticoagulant routine health education interventions from July 2018 to December 2018 were used as controls In the group,160 patients who received the intervention of the phased missionary continuous quality improvement project activity from January 2019 to June 2019 were used as the observation group.The follow-up was followed for 6 months,and the statistical comparison and analysis results of the questionnaire survey on the knowledge and behavior of warfarin drug anticoagulant therapy and the satisfaction evaluation of special activities were compared between the two groups.Results:The patients were followed up for six months after discharge and found that there were significant differences between the two groups of patients on anticoagulant drug treatment,knowledge and behavior,and the evaluation of satisfaction with specific activities.The follow-up compliance and health missionary satisfaction were significantly better than the control group,the difference was obvious and statistically significant(P<0.05).Conclusion:Warfarin staged missionary continuous quality improvement project activities have a greater impact on the knowledge and behavior of patients taking anticoagulant drugs after cardiac and vascular surgery.The awareness rate of anticoagulant drugs has increased significantly,and postoperative anticoagulation complications have been significantly reduced to improve the safety of surgical treatment of patients,patient satisfaction has been significantly improved.
