完全腹腔镜联合纤维胆道镜保胆取石术在胆囊结石中的应用效果

Analysis of the effect and complication rate of complete laparoscopy combined with fiberoptic choledochoscope for gallstone preservation and removal of gallbladder stones

目的探讨完全腹腔镜联合纤维胆道镜保胆取石术在胆囊结石中的应用效果。方法选取2018年1月至2020年1月的北京京煤集团总医院收治的70名胆囊结石患者作为研究对象,采用计算机随机分组法将其分为观察组(35例)和对照组(35例),观察组采用腹腔镜联合纤维胆道镜保胆取石术治疗,对照组采用腹腔镜胆囊切除术治疗。比较两组患者的手术用时、术中出血量、术后镇痛泵使用时间、住院总时间以及术后并发症总发生率。结果观察组的手术用时长于对照组,术中出血量少于对照组,术后镇痛泵使用时间、住院总时间短于对照组,差异有统计学意义(P<0.05);术后对患者进行6个月随访,观察组的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论完全腹腔镜联合纤维胆道镜保胆取石术治疗胆囊结石的效果显著,并发症总发生率低,方案值得借鉴。

Objective To analyze the effect and complication rate of complete laparoscopy combined with fiberoptic choledochoscope for gallstone preservation and removal of gallbladder stones.Methods A total of 70 patients with gallbladder stones admitted to the Beijing Jingmei Group General Hospital from January 2018 to January 2020 were selected as the research objects,and they were divided into observation group(35 cases)and control group(35 cases)by computer randomization method.The observation group was treated with laparoscopy combined with fiberoptic choledochoscopy to preserve gallstones,and the control group was treated with laparoscopic cholecystectomy.The operation time,intraoperative blood loss,postoperative analgesic pump use time,total hospital stay and the total incidence of postoperative complications were compared between the two groups.Results The operation time of the observation group was longer than the control group,the amount of intraoperative blood loss was less than the control group,and the postoperative analgesic pump use time and total hospitalization time were shorter than the control group,the differences were statistically significant(P<0.05).The patients were followed up for 6 months after operation,and the total incidence of complications in the observation group was lower than that in the control group,with statistical significance(P<0.05).Conclusion Complete laparoscopy combined with fiberoptic choledochoscope is effective in the treatment of gallbladder stones with a low complication rate,the program is worth learning.
