摘要
目的探究活动性巨细胞病毒感染DNA载量与2型糖尿病(T2DM)患者T淋巴细胞亚群的关系。方法回顾性分析2019年10月至2020年5月萍乡市人民医院收治的80例T2DM患者的临床资料,根据活动性巨细胞病毒感染情况将其分为观察组(巨细胞病毒感染阳性,病毒DNA载量>500拷贝数/ml,n=18)和对照组(巨细胞病毒感染阴性,病毒DNA载量≤500拷贝数/ml,n=62)。比较两组T淋巴细胞亚群(CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))、胰岛功能[空腹C肽(FCP)、餐后2小时C肽(F2CP)水平差异,采用Pearson相关性分析法分析活动性巨细胞病毒感染DNA载量与T淋巴细胞亚群水平和胰岛功能的相关性。结果观察组CD4^(+)细胞水平、CD4^(+)/CD8^(+)低于对照组,CD8^(+)细胞水平高于对照组,差异有统计学意义(P<0.05)。观察组FCP、F2CP水平低于对照组,差异有统计学意义(P<0.05)。巨细胞病毒感染DNA载量与CD4^(+)、CD4^(+)/CD8^(+)、FCP、F2CP呈负相关,与CD8^(+)水平呈正相关(P<0.05)。结论活动性巨细胞病毒感染DNA载量与T2DM患者T淋巴细胞亚群关系密切,患者体内巨细胞病毒越活跃,其体内免疫失衡程度越大,胰岛功能损伤也越严重,可采取相应治疗手段,改善巨细胞病毒感染情况。
Objective To explore the relationship between DNA load of active cytomegalovirus infection and T lymphocyte subsets in patients with type 2 diabetes mellitus(T2DM).Methods The clinical data of 80 patients with T2DM treated in Pingxiang People′s Hospital between October 2019 and May 2020 were retrospectively analyzed.According to the active cytomegalovirus infection status,the patients were divided into observation group(positive cytomegalovirus infection,viral DNA load>500 copies/ml,n=18)and control group(negative cytomegalovirus infection,viral DNA load≤500 copies/ml,n=62).The T lymphocyte subsets(CD4^(+),CD8^(+),CD4^(+)/CD8^(+))and pancreatic islet function(fasting C-peptide[Fins],fasting 2 h C-peptide[F2CP])were compared between the two groups.Pearson correlation analysis was used to analyze the correlation between DNA load of active cytomegalovirus infection and levels of T lymphocyte subsets and pancreatic islet function.Results The level of CD4^(+)cells and CD4^(+)/CD8^(+)in observation group were significantly lower than those in control group,while the level of CD8^(+)cells was higher than that in control group,the differences were statistically significcant(P<0.05).The levels of FCP and F2CP were lower in observation group than those in control group,the differences were statistically significcant(P<0.05).The DNA load of cytomegalovirus infection was negatively correlated with CD4^(+),CD4^(+)/CD8^(+),Fins and ISI,and was positively correlated with CD8^(+)level(P<0.05).Conclusion The DNA load of active cytomegalovirus infection is closely related to the T lymphocyte subsets of patients with T2DM.The more active cytomegalovirus in the body of patients,the greater the degree of body′s immune imbalance and the more severe the islet function damage.It is necessary to take corresponding treatment methods to improve the cytomegalovirus infection status.
