摘要
目的探讨照护者协同下跨理论模型健康教育在脑卒中偏瘫患者康复中应用效果。方法选取2019年1月至2020年6月九江市第一人民医院神经内科70例脑卒中偏瘫患者作为研究对象,采用随机数字表法分为对照组(35例)、观察组(35例),对照组采用传统健康教育,观察组采用照护者协同下跨理论模型健康教育,比较两组患者护理前后康复效果、自我效能、自我管理行为能力及生活自理能力。结果护理前两组的Fugl-Meyer运动功能评分法(FMA)评分及改良爱丁堡斯堪的纳威亚卒中量表(MESSS)评分比较,差异无统计学意义(P>0.05);护理后,两组患者FMA评分高于护理前,MESSS评分低于护理前,且观察组FMA评分高于对照组,MESSS评分低于对照组,差异有统计学意义(P<0.05);护理前两组的慢性病自我效能量表(CDSES)评分、自我管理行为评分及生活自理能力[Barthel指数(BI)]评分比较,差异无统计学意义(P>0.05);护理后,两组患者CDSES评分、自我管理行为评分及BI评分均高于护理前,且观察组CDSES评分、自我管理行为评分及BI评分均高于对照组,差异有统计学意义(P<0.05)。结论对脑卒中偏瘫患者康复护理中,采用照护者协同跨理论模型健康教育干预,可提升康复效果,提升患者自我效能、自我管理行为能力,同时可提升患者生活自理能力,效果理想。
Objective To explore the effect of cross-theoretical model health education under caregiver coordination on the rehabilitation in patients with stroke hemiplegia.Methods A total of 70 patients with stroke hemiplegia treated in the Department of Neurology in the Jiujiang First People′s Hospital from January 2019 to June 2020 were selected.They were divided into the control group(35 patients)and the observation group(35 patients)using random number table method.The control group were given traditional health education,and the observation group were given cross-theoretical model health education under caregiver coordination.The rehabilitation effect,self-efficacy,self-management behavior ability and self-care ability before and after intervention were compared between the two groups.Results There were no significant differences in scores of Fugl-Meyer assessment(FMA)and modified Edinburgh scandinavian stroke scale(MESSS)between the two groups before intervention(P>0.05).After intervention,the scores of FMA in both groups were higher than those before intervention,and the scores of MESSS in both groups were lower than those before intervention.The score of FMA in the observation group was higher than that in the control group,and the score of MESSS in the observation group was lower than that in the control group,with statistically significant differences(P<0.05).There were no significant differences in scores of Chronic disease self-efficacy scale(CDSES),self-management behavior and self-care ability(Barthel index[BI])between the two groups before intervention(P>0.05).After intervention,the scores of CDSES,self-management behavior and BI in both groups were higher than those before intervention.The scores of CDSES,self-management behavior and BI in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).Conclusion The application of cross-theoretical model health education under caregiver coordination can improve rehabilitation effect,and enhance patients′self-efficacy,self-management behavior ability,and self-care ability in the rehabilitation of patients with stroke hemiplegia.The effect is ideal.
