快速康复外科理念围手术期护理在老年脆性髋部骨折患者中的应用

Application of fast track surgery concept perioperative nursing in elderly patients with brittle hip fracture

目的分析快速康复外科(FTS)理念的围手术期护理在老年脆性髋部骨折患者的应用价值。方法选取2019年1月至2020年8月孝感市中心医院收治的80例老年脆性髋部骨折患者作为研究对象,采用抽签法将其分为对照组和观察组,每组各40例。对照组采用常规护理,观察组采用常规护理+FTS理念的围手术期护理干预,比较两组护理前、护理后1个月的髋关节功能(Harris)、疼痛评分(VAS)、日常生活能力(ADL),并发症发生率及随访结果。结果护理前,两组的Harris、VAS、ADL评分比较,差异无统计学意义(P>0.05);护理后1个月,两组的Harris、ADL评分高于护理前,VAS评分低于护理前;且观察组的Harris、ADL评分高于对照组,VAS评分低于对照组,差异均有统计学意义(P<0.05)。观察组的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。随访半年,两组均未出现病情复发不良事件;两组随访结果比较,差异无统计学意义(P>0.05)。结论老年脆性髋部骨折应用FTS理念围手术期护理,可改善患者髋关节功能,缓解疼痛,提高日常生活能力,降低并发症发生率,效果显著。

Objective To analyze the application value of perioperative nursing intervention of rapid recovery surgery(FTS)in elderly patients with Brittle hip fracture.Methods A total of 80 elderly patients with Brittle hip fracture admitted to Xiaogan Center Hospital from January 2019 to August 2020 were selected as study subjects.The patients were divided into the control group and the observation group by drawing lots,40 cases in each group.The control group was given routine nursing,the observation group was given routine nursing plus FTS perioperative nursing intervention,and the hip function(Harris),visual aralogue scale(VAS),activity of dailylivzing(ADL),incidence of complication,and the follow-up results were compared before and 1 month after nursing,incidence of complications and follow-up results.Results Before nursing,there were no significant differences in Harris,VAS and ADL scores between the two groups(P>0.05);at 1 month after nursing,the Harris and ADL scores of two groups were higher than those before nursing,the VAS scores of two groups were lower than those before nursing,the Harris and ADL score of the observation group were higher than those of the control group,and the VAS score of the observation group was lower than that of the control group,the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).The patients were followed up for half a year,and there was no recurrence of adverse events in the two groups.There was no significant difference in the follow-up results between the two groups(P>0.05).Conclusion The perioperative nursing of FTS concept in elderly patients with brittle hip fracture can improve hip joint function,relieve pain,improve daily living ability,and reduce the incidence of complications.
