摘要
目的研究老年冠心病患者脉搏波传导速度(PWV)相关参数与心脏结构和功能的相关性,为老年冠心病患者的预后判断,提供相应的理论依据。方法随机选取100例冠心病患者和100例健康对照(健康对照组)。采用自动PWV分析仪测定所有研究对象的颈动脉-股动脉、颈动脉-桡动脉PWV值(CF-PWV、CR-PWV),超声心动图常规检查心脏结构及血流并测定左心功能,分析2组的相关性。结果2组人群中年龄、性别以及LVPW分布差异无统计学意义(P>0.05),血压、心脏结构参数(LAD和LVDD)、心脏功能参数(LVEF和E/A)以及PWV参数(CF-PWV和CR-PWV)等在2组人群中分布差异有统计学意义(P<0.05);CF-PWV与心脏结构及功能参数的均存在相关性(P均<0.05)。其中,CF-PWV与LAD、LVDD、LVPW呈正相关,与LVEF、E/A呈负相关。CR-PWV与心脏结构参数中的LVDD、LVPW呈正相关(P均<0.05),与LAD不存在相关性(P>0.05);CR-PWV与心脏功能各参数不存在相关性(P均>0.05)。多元线性逐步回归分析结果显示,CF-PWV的独立相关因素是LVDD和LVPW,CR-PWV的独立相关因素是LVPW和是否患有冠心病。结论PWV相关参数与心脏结构和功能有一定的相关性,提示老年冠心病检测相应指标,有助于对此类患者早期进行积极干预和有效治疗,可做为判断老年冠心病患者风险及发生临床事件的有用指标。
Objective To investigate the effects of To observe the correlation between PWV and cardiac structure and function in patients with coronary heart disease,so as to provide more evidence for predicting the prognosis of patients with coronary heart disease.Methods 100 patients with coronary heart disease and 100 healthy controls were randomly selected.Carotid-femoral artery PWV values(CF-PWV and CR-PWV)were measured by automatic PWV analyzer.Cardiac structure and blood flow were examined by echocardiography routinely and left ventricular function was measured.Results This study included 100 cases of coronary heart disease group and 100 cases of healthy control group.There was no significant difference in age,sex and LVPW distribution between the two groups(P>0.05).Blood pressure,cardiac structural parameters(LAD and LVDD),cardiac function parameters(LVEF and E/A)and PWV parameters(CF-PWV and CR-PWV)were significantly different between the two groups(P>0.05).There were correlations among all of them(P>0.05).CF-PWV is positively correlated with LAD,LVDD and LVPW,but negatively correlated with LVEF and E/A.CR-PWV was positively correlated with LVDD and LVPW in cardiac structural parameters(P>0.05),but not with LAD(P>0.05);CR-PWV was not correlated with cardiac function parameters(P>0.05).Multivariate linear stepwise regression analysis showed that the independent related factors of CF-PWV were LVDD and LVPW,while the independent related factors of CR-PWV were LVPW and coronary heart disease.Conclusion Detection of PWV in patients with risk factors of coronary heart disease,early coronary heart disease and severe coronary events is helpful for active intervention and effective treatment.
