横分纵扎术治疗重度环状混合痔的临床疗效分析

Transverse Dividing & Longitudinal Ligature in the Treatment of Severe Circumferential Mixed Hemorrhoids:Clinical Effect Analysis

为探讨横分纵扎术治疗重度环状混合痔的临床疗效,将2018年2月至2020年2月我科收治的86例重度环状混合痔患者随机分为观察组和对照组,各43例,观察组行横分纵扎术治疗,对照组行外剥内扎术治疗,比较2组临床总体疗效、创面愈合时间、术后并发症等。结果显示,2组患者临床总有效率、创面愈合时间比较差异无统计学意义,P>0.05。观察组患者术后疼痛程度明显轻于对照组,创缘水肿发生率明显低于对照组,P <0.05;2组术后尿潴留及肛门狭窄发生率比较差异无统计学意义,P>0.05。2组术后均无大出血病例。观察组术后30d残余外痔覆盖点位数明显少于对照组,P <0.05。结果表明,横分纵扎术治疗重度环状混合痔临床总体疗效与外剥内扎术相当,创面愈合时间相近,但前者术后疼痛程度更轻、创缘水肿发生率更低、外痔残留更少,患者痛苦更小。

This study was to investigate the effect of transverse dividing &longitudinal ligature for treating severe circumferential mixed hemorrhoids(SCMH),randomly divided 86 SCMH patients treated in author’s department from 2018-02 to 2020-02 into observation group(43 cases,received transverse dividing &longitudinal ligature)and control group(43 cases,external dissection &internal ligature);then compared both groups’ whole effect,wound healing time,and postoperative complication,etc.As results,in total effective rate,and in wound-surface healing time there was no statistical difference between the two groups(P<0.05);in postoperative pain intensity,and in wound-edge edema incidence observation group was significantly and respectively milder,and lower than control group(P <0.05);after surgery both groups’ incidence of uroschesis and anal stenosis had no significant difference(P >0.05),but 30 days after surgery the sites covered by residual external hemorrhoids of observation group were significantly less than that of control group(P <0.05);after surgery no cases of massive bleeding were found in both groups.Results show that the clinical whole effect of transverse dividing &longitudinal ligature for SCMH is as same as external dissection &internal ligature,and wound-surface healing time of both groups is close,but the former(procedure)’s postoperative pain is more milder;incidence of wound-edge edema,more lower;external hemorrhoids residuals,less;as well as patients’ sufferings,more smaller.
