2018年-2020年某基层医疗社区医院口服调节血脂药使用情况分析

Analysis on the use of oral lipid regulating drugs in a primary medical community hospital from 2018 to 2020

目的:观察2018年-2020年厦门市思明区筼筜街道社区卫生服务中心口服调节血脂药使用情况及变化规律,为临床合理用药和管控提供参考。方法:通过厦门市基层医疗卫生信息系统云平台提取2018年-2020年口服调节血脂药相关使用数据,对各类药品的销售金额、限定日剂量(DDD)、用药频度(DDDs)、日均费用(DDC)和排序比(B/A)等进行回顾性统计分析。结果:2018年-2020年口服调节血脂药中,他汀类药物的销售金额、DDDs连续3年位居首位,其次是贝特类。具体药物中阿托伐他汀连续3年排名第1位;瑞舒伐他汀、普伐他汀、辛伐他汀和普罗布考下降幅度较大;氟伐他汀、匹伐他汀和依折麦布呈上升趋势;非诺贝特和阿昔莫司相对平稳。“4+7城市”带量采购实施两年来,某基层医院口服调节血脂药的DDC呈逐年降低趋势。2018年-2020年大部分药品的B/A值都在1.00左右,同步性较好。结论:2018年-2020年降脂药物的选择和使用相对合理,患者用药情况符合指南要求;“4+7城市”带量采购、第二轮和第三轮带量采购的实施使患者的药品费用大幅下降。

Objective:To understand the use and change of oral lipid regulating drugs in community health service center of yundang street in siming district of xiamen from 2018 to 2020,so as to provide reference for clinical rational drug use and management.Methods:Through the`cloud platform of Xiamen primary medical and health information system',the data of oral lipid regulating drugs in our hospital from 2018 to 2020 were extracted,and the sales amount,defined daily dose(DDD),defined daily doses(DDDs),defined daily cost(DDC)and sorting ratio(B/A)of various drugs were retrospectively analyzed.Results:Among the oral lipid regulating drugs in hospital from 2018 to 2020,the sales sum of statins and DDDs ranked first for three consecutive years,followed by Bates.Among the specific drugs,atorvastatin ranked first for three consecutive years,rosuvastatin,pravastatin,simvastatin and probucol decreased significantly;fluvastatin,pitavastatin and ezetimibe showed an upward trend;fenofibrate and acipimox were relatively stable.In the two years since the implementation of`4+7 cities'procurement,the DDC of oral lipid regulating drugs in hospital has been decreasing year by year.From 2018 to 2020,the B/A value of most drugs is about 1.00,with good synchronization.Conclusion:From 2018 to 2020,the selection and use of lipid-lowering drugs in our hospital are relatively reasonable,and the drug use of patients meets the requirements of the guidelines.With the implementation of`4+7 cities'with volume procurement,the second round and the third round of volume procurement,the drug cost of patients has decreased significantly.
