1281株奇异变形杆菌耐药率与抗菌药物用药频度的相关性分析 被引量:5

摘要 目的:分析奇异变形杆菌的检出情况、耐药变迁及与同期抗菌药物用药频度(DDDs)之间的相关性,为临床抗感染提供参考。方法:对2013~2020年我院奇异变形杆菌的耐药率、同期抗菌药物DDDs变化情况进行回顾性分析,运用多重线性回归模型分析耐药情况及与同期抗菌药物DDDs间是否存在线性相关,找出相关因素。结果:我院临床分离的1 281株奇异变形杆菌主要来源于痰液和尿液,分别占40.13%和34.58%。奇异变形杆菌对氨苄西林的耐药率较高,对哌拉西林/他唑巴坦、头孢西丁和美罗培南耐药率较低。多重线性回归分析结果显示,奇异变形杆菌对头孢克肟的耐药率只与阿米卡星的DDDs呈正向的线性相关,其回归系数β为0.059。此外,奇异变形杆菌对左氧氟沙星的耐药率与美罗培南DDDs呈正向的线性相关;其对头孢吡肟的耐药率与阿米卡星DDDs呈正向的线性相关;其对哌拉西林/他唑巴坦及美罗培南的耐药率与庆大霉素DDDs均呈负向的线性相关。结论:奇异变形杆菌的耐药率与抗菌药物DDDs间存在一定的相关性,临床应合理规范使用抗菌药物。 Objective:to analyze the detection of Proteus mirabilis, the change of drug resistance and the correlation with DDDs of antibiotics in the same period, so as to provide reference for clinical anti infection.Methods: The drug resistance of Proteus mirabilis isolated and the DDDs of antibiotics in our hospital from 2013 to 2020 were analyzed retrospectively. Correlations between drug resistance of Proteus mirabilis and the DDDs of antibioticswere analyze by multiple linear regression model whether there was linear correlation and find out the relevant factors. Results:1 281 strains of Proteus mirabilis were mainly isolated from sputum and urine, accounting for 40.13% and 34.58% respectively. Proteus mirabilis was highly resistant to ampicillin and lowly resistant to piperacillin-tazobactam, cefoxitin and meropenem. Multiple linear regression showed that the resistance rate of Proteus mirabilis to cefixime was only positively correlated with DDDs of amikacin, and the regression coefficient β was 0.059. In addition, the results of linear regression analysis showed that the resistance rate of Proteus mirabilis to levofloxacin was positively correlated with meropenem DDDs;The resistance rate to cefepime was positively correlated with amikacin DDDs;The resistance rates to piperacillin-tazobactam and meropenem were negatively correlated with gentamicin DDDs. Conclusion: There is a certain correlation between the drug resistance rate of Proteus mirabilis and DDDs, and the clinical use of antibiotics should be reasonable and standardized.
