摘要
目的探究分析中药颗粒配方治疗气阴两虚型早期糖尿病肾脏病的安全性及围治疗期miRNA-92a(miR-92a)的变化情况。方法收集2014年6月—2015年8月医院收治的气阴两虚型早期糖尿病肾病患者共84例,根据治疗方案不同分为两组,其中对照组(36例)采用基本降糖治疗:即口服格列喹酮,控制不理想的患者,可加用瑞格列奈或阿卡波;研究组(48例)在基本降糖的基础上,采用消渴保肾颗粒治疗。结果与对照组患者相比,研究组患者的治疗总有效率明显更高(P<0.05),治疗后研究组患者的尿微量蛋白和尿素氮水平明显更低(P<0.05),血糖水平和血肌酐水平两组间差异无统计学意义(P>0.05);治疗前,两组患者血清中miR-92a水平差异无统计学意义(P>0.05);治疗后,与对照组患者相比,研究组患者血清miR-92a水平明显更低(P<0.05);且所有患者均未发生不良反应。结论消渴保肾颗粒治疗气阴两虚型早期糖尿病肾病疗效显著,可明显降低患者血清miR-92a的水平,安全可靠。
Objective To explore and analyze the safety of traditional Chinese medicine granules treating early diabetic nephropathy with deficiency of both Qi and Yin and the changes of miRNA-92 a(miR-92 a) during the peri-treatment period. Methods A total of 84 patients with early diabetic nephropathy of Qi and Yin deficiency type admitted to our hospital from June 2014 to August 2015 were collected. They were divided into two groups according to different treatment plans. The control group(36 cases) received basic hypoglycemic treatment: oral gliquidone. Patients with unsatisfactory control can be added with repaglinide or acarb. The study group(48 cases) used Xiaoke Baoshen Granule(消渴保肾颗粒) on the basis of basic hypoglycemic treatment. Results Compared with patients in the control group, the total effective rate of the study group was significantly higher(P<0.05). After treatment, the levels of urine microprotein and urea nitrogen in the study group were significantly lower(P<0.05), and there was no difference in the blood glucose level or creatinine levels between the two groups(P>0.05). Before treatment, there was no difference in serum miR-92 a levels between the two groups(P>0.05). After treatment, compared with that of the control group, the study group’s serum miR-92 a level was significantly lower(P<0.05) and all patients had no adverse reactions. Conclusion The curative effect of Xiaoke Baoshen Granule on patients with early diabetic nephropathy with deficiency of Qi and Yin is significant, which can significantly reduce the level of serum miR-92 a in patients. It is safe and reliable.
