苇茎汤加味对慢性阻塞性肺疾病急性加重期炎性因子和免疫因子影响机制的研究 被引量:12

摘要 目的研究苇茎汤加味治疗慢性阻塞性肺疾病急性加重期对患者炎症和免疫功能的影响效果,分析相关可能机制。方法抽选出2019年3月—2020年2月收入住院治疗的慢性阻塞性肺疾病急性加重期患者90例开展调研,采用统计学随机数字表方法分为研究组(45例,常规治疗+苇茎汤加味治疗)和对照组(45例,常规治疗),治疗2个疗程后比较两组呼吸功能、炎性因子和免疫因子变化。结果治疗2个疗程后,研究组动脉血气氧合指数(PO_(2)/FiO_(2))、氧分压(PaO_(2))水平显著高于对照组,二氧化碳分压(PaCO_(2))水平显著低于对照组;炎性因子白细胞(WBC)、C反应蛋白(CRP)、降钙素原(PCT)水平均显著低于对照组;免疫因子CD_(4)^(+)、IFN-γ、IFN-γ/IL-4水平高于对照组,CD+8、CD_(4)^(+)CD+25Treg、IL-4水平低于对照组,差异有统计学意义(P<0.05)。结论苇茎汤加味治疗慢性阻塞性肺疾病急性加重期可通过调节免疫平衡,控制炎症反应,从而显著改善患者呼吸功能,值得临床推广。 Objective To study the effect of Weijing Decoction(苇茎汤) on inflammation and immune function in patients with acute exacerbation of chronic obstructive pulmonary disease(COPD) and analyze the possible mechanism. Methods Ninety patients with severe pneumonia were selected from March 2019 to February 2020. They were divided into study group(45 cases, routine treatment+Weijing Decoction) and control group(45 cases, regular treatment). After two courses of treatment, the changes of respiratory function, inflammatory factors and immune factors were compared. Results After two courses of treatment, the levels of PO_(2)/FiO_(2) and PaO_(2) in the study group were significantly higher than those in the control group, and the level of PaCO_(2) was significantly lower than that in the control group. The levels of WBC, CRP and PCT were significantly lower than those in the control group. The levels of CD_(4)^(+), IFN-γ and IFN-γ/IL-4 were higher than that in the control group, and the levels of CD+8, CD_(4)^(+)CD+25Treg and IL-4 were lower than those in the control group, with statistical significance(P<0.05). Conclusion Weijing Decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease can significantly improve the respiratory function of patients by regulating the immune balance and controlling the inflammatory response, worthy of clinical promotion.
