止血镇痛胶囊联合瑞芬太尼治疗剖宫产术后镇痛的疗效及对血清5-HT、PRL水平影响

Effect of Zhixue Zhentong Capsule(止血镇痛胶囊)Combined with Remifentanil on Postoperative Analgesia after Cesarean Section and Its Effect on Serum Levels of 5-HT and PRL

目的探讨止血镇痛胶囊联合瑞芬太尼治疗剖宫产术后镇痛的疗效及对血清5-HT、PRL水平影响。方法选取来医院接受治疗的110例剖宫产术后患者,随机分为两组各55例。对照组患者给予瑞芬太尼,试验组在对照组基础上口服止血镇痛胶囊。对比两组患者的临床疗效、镇痛效果和下肢运动神经阻滞程度、血清皮质醇水平及SpO2、MAP、HR值、血液流变学指标及血清IL-6、CRP和TNF-α水平。结果治疗后,试验组总有效率明显较高(P<0.05);两组患者治疗前镇痛效果和下肢运动神经阻滞程度比较差异无统计学意义(P>0.05);治疗后两组镇痛效果和下肢运动神经阻滞程度显著降低(P<0.05);并且试验组降低较明显(P<0.05);两组患者治疗前血清皮质醇水平比较差异无统计学意义(P>0.05);两组患者治疗后血清皮质醇水平显著降低(P<0.05),并且试验组血清皮质醇水平明显低于对照组(P<0.05);两组SpO2、MAP、HR值治疗前后比较差异无统计学意义(P>0.05);两组患者治疗前HBV、FIB、LBV和血浆黏度水平比较差异无统计学意义(P>0.05);治疗后两组患者HBV、FIB、LBV和血浆黏度水平显著降低(P<0.05);并且试验组降低较明显(P<0.05);两组患者治疗前血清IL-6、CRP和TNF-α水平比较差异无统计学意义(P>0.05);两组患者治疗后血清IL-6、CRP和TNF-α水平明显降低(P<0.05);且试验组降低较明显(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05);两组患者治疗前血清5-HT、PRL水平比较差异无统计学意义(P>0.05);治疗后两组血清5-HT、PRL水平显著降低(P<0.05);并且试验组降低较明显(P<0.05)。结论采用止血镇痛胶囊联合瑞芬太尼用于治疗剖宫产术后镇痛具有较好的镇痛效果,安全性较高,值得在临床上推广应用。

Objective To investigate the efficacy of Zhixue Zhentong Capsule(止血镇痛胶囊) combined with remifentanil in the treatment of postoperative analgesia after cesarean section and its effect on serum levels of 5-HT and PRL. Methods A total of 110 patients who received treatment after cesarean section in our hospital were randomly divided into two groups, 55 patients in each group. Patients in the control group were given remifentanil, and the experimental group was given Zhixue Zhentong Capsule orally on the basis of the control group. The clinical efficacy, analgesic effect, degree of lower limb motor nerve block, serum cortisol level, SpO2, MAP, HR value, hemorheology index and serum levels of IL-6, CRP and TNF-α were compared between the two groups. Results After treatment, the total effective rate of the experimental group was significantly higher(P<0.05). There was no difference in the analgesic effect or the degree of lower limb motor nerve block before treatment between the two groups(P>0.05). After treatment, the analgesic effect and the degree of lower limb motor nerve block were significantly reduced in the two groups(P<0.05). In the experimental group, the decrease was more significant(P<0.05). There was no difference in serum cortisol levels between the two groups before treatment(P>0.05). After treatment, the serum cortisol level of the two groups was significantly reduced(P<0.05), and the serum cortisol level of the experimental group was significantly lower than that of the control group(P<0.05). SpO2, MAP and HR of the two groups showed no difference before and after treatment(P>0.05). There was no difference in HBV, FIB, LBV or plasma viscosity between the two groups before treatment(P>0.05). After treatment, the levels of HBV, FIB, LBV and plasma viscosity in the two groups were significantly decreased(P<0.05). In the experimental group, the decrease was more significant(P<0.05). There was no difference in serum levels of IL-6, CRP and TNF-α between the two groups before treatment(P>0.05). After treatment, the serum levels of IL-6, CRP and TNF-α were significantly decreased in both groups(P<0.05). In the experimental group, the reduction was more significant(P<0.05). There was no difference in the incidence of adverse reactions between the two groups(P>0.05). There was no difference in serum levels of 5-HT or PRL between the two groups before treatment(P>0.05). After treatment, serum levels of 5-HT and PRL in both groups were significantly decreased(P<0.05). In addition, the levels of 5-HT and PRL were significantly reduced in the experimental group(P<0.05). Conclusion Zhixue Zhentong Capsule combined with remifentanil has a good analgesic effect, high safety in the treatment of postoperative analgesia after cesarean section, and is worthy of clinical application.
