阳和汤联合唑来膦酸治疗肺癌骨转移癌痛的疗效及对血清NTX、ALP的影响

Therapeutic effect of Yanghe decoction combined with zoledronic acid on bone metastasis of lung cancer and changes of serum NTX and ALP levels

目的探讨阳和汤联合唑来膦酸治疗肺癌骨转移瘤癌痛的疗效及对血清NTX、ALP的影响。方法采用完全随机法将84例患者随机分为对照组和观察组,各42例。对照组予以唑来膦酸治疗,观察组在对照组的基础上加用阳和汤。通过对比治疗前后癌痛的疗效、疼痛VAS评分、血清NTX及ALP水平、卡氏功能(KPS)评分、不良反应等,比较两组治疗方法的疗效差异。结果治疗后两组癌痛治疗有效率比较,观察组为78.5%,对照组为54.7%,差异有统计学意义(P<0.05),治疗后两组患者疼痛VAS评分均较治疗前下降,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后观察组血清骨代谢标志物NTX、ALP水平较对照组明显下降,差异有统计学意义(P<0.05),治疗后两组患者KPS评分比较,观察组有效率为88.09%,对照组有效率为80.95%,差异无统计学意义(P>0.05)。两组患者不良反应比较,差异无统计学意义(P>0.05)。结论阳和汤联合唑来膦酸能够有效缓解肺癌骨转移癌痛,并降低血清NTX、ALP水平。

ObjectiveTo explore the curative effects of Yanghe decoction combined with zoledronic acid in the treatment of cancer pain caused by bone metastasis of lung cancer and its influence on the level of serum levels of N-terminal telopeptide of type I collagen(NTX) and alkaline phosphatase(ALP).MethodsA total of 84 patients were randomly divided into two groups, 42 in the control group and 42 in the observation group. The control group was treated with zoledronic acid, and the observation group was treated with Yanghe Decoction plus zoledronic acid. Compare the curative effects on cancer pain, the visual analog scale(VAS) scores before and after treatment, changes in serum NTX and ALP levels, Karnofsky(KPS) scores, and adverse reactions between the two groups.ResultsAfter treatment, the pain remission rate was78.5% in the observation group and 54.7% in the control group, with statistical difference between the two groups(P<0.05).The VAS score of pain in the two groups after treatment was lower than before treatment(P<0.05), and it was lower in observation group than in control group(P<0.05). The serum levels of bone metabolism markers NTX and ALP in the observation group were significantly lower than those in the control group(P<0.05). In the comparison of KPS scores after treatment, the effective rate was 88.09% in the observation group and 80.95% in the control group, and there was no statistically significant difference between the two groups(P>0.05).ConclusionYanghe decoction combined with zoledronic acid can effectively relieve the pain caused by bone metastasis from lung cancer and reduce the serum levels of NTX and ALP.
