持久性隆起性红斑合并边缘性角膜溃疡1例并文献复习

A Case of Erythema Elevatum Diutinum Associated with Peripheral Ulcerative Keratitis and Literature Review

患者女,18岁,耳后、四肢暗红色结节,伴视物模糊,膝关节疼痛3年。双眼裂隙灯前节照相示:边缘性角膜溃疡;实验室检查:类风湿因子(rheumatoid factor,RF)阴性;皮损组织病理示:皮肤浅中层血管纤维化。诊断:持久性隆起性红斑合并边缘性角膜溃疡。给予口服羟氯喹、米诺环素及复方倍他米松注射液局部封闭,他克莫司滴眼液、环孢素滴眼液点双眼等治疗后,双眼视力及皮损均得到改善。

An 18-year-old female presented with a 3-year history of dark red nodules on the posterior ears,arms and legs,accompanied with decreased vision and joint pains in the knees.Slit lamp photography showed peripheral ulcerative keratitis.Laboratory investigations showed a negative RF(rheumatoid factor).Skin biopsy revealed vascular fibrosis in the superficial and middle dermis.She was diagnosed with erythema elevatum diutinum associated with peripheral ulcerative keratitis.Treatment consists of oral minocycline and hydroxychloroquine as well as intralesional injection of compound betamethasone.Topical tacrolimus eye drops or cyclosporine eye drops was also administered.Visual acuity and skin lesions of the patient improved.
