逻辑回归分析评估手臂对盆腔血管3D-DSA成像质量的影响

摘要 目的:评估手臂放在身体两侧对盆腔动脉3D-DSA成像中图像质量的影响。方法:对2016年1月-2020年11月盆腔动脉栓塞术中获得3D-DSA图像的91例患者进行回归分析,其中手臂放于身体两侧的为观察组(48例),手臂举过头顶的为对照组(43例)。2名介入医生回顾性分析所有图像,并将血管成像评分从1分到5分,范围从无法诊断到最佳显示。放射条纹伪影评分为1~3分。采集过程中观察呼吸运动、身体质量指数(BMI)和辐射面积乘积(DAP)评估手臂位置对血管成像和伪影的影响。结果:在盆腔动脉3D-DSA成像中,血管成像与手臂位置无明显相关性。DAP组间差异无统计学意义(P=0.54)。结论:盆腔动脉3D-DSA血管成像与手臂位置无明显相关性,在手术过程中无需特意将手臂举过头顶以得到更高图像质量。 Objective:To evaluate the effect of arms placed by the sides of the body on the image quality in 3D-DSA imaging of pelvic arteries.Methods:A total of 91 patients with 3D-DSA images obtained during pelvic artery embolization from Jan 2016 to Nov 2020 were analyzed.48 patients in the observation group(arms down)and 43 patients in the control group(arms up).Two interventional physicians retrospectively analyzed all images and rated angiography scores on a scale of 1 to 5,ranging from undiagnosed to optimal.Streak artifacts across axial images were rated from 1 to 3 based on resulting image quality.Presence of respiratory during acquisition,body mass index and radiation dose area product(DAP)were also recorded and analyzed.Univariate and multivariate analyses were used to assess the impact of arm position on vascular imaging and imaging artifacts.Results:vascular imaging were not significantly associated with arm position during 3D-DSA imaging of pelvic artery.DAP was not statistically different between the groups(26.1 Gy cm2[4.2-102.6]arms down,23.9 Gy cm2[6.1-73.4]arms up,P=0.54).Conclusion:vascular imaging were not significantly associated with arm position during 3D-DSA imaging of pelvic artery.Arming up is not needed to get better images during the procedure.
