九华痔疮栓联合乳果糖口服液、培菲康治疗小儿功能性便秘伴血便的临床疗效

Clinical efficacy of Jiuhua hemorrhoids suppository plus lactulose solution and bifico in the treatment of children with functional constipation combined with hematochezia

目的探讨九华痔疮栓联合乳果糖口服液、培菲康治疗小儿功能性便秘及肛裂引发的血便的临床疗效。方法选择我院2017年3月~2019年3月门诊符合小儿功能性便秘出现肛裂伴血便的儿童63例,随机分为两组:治疗组32例:给予九华痔疮栓联合口服乳果糖口服液、培菲康治疗。对照组31例:给予口服乳果糖口服液联合培菲康治疗。两组均改善不良饮食习惯及排便训练,药物疗程均2周。结果两组功能性便秘(functional constipation, FC)儿童在治疗3天后,治疗组便秘总有效率为75.0%,对照组便秘总有效率为74.1%,两组在便秘早期疗效相当(P>0.05);治疗组肛裂伴肉眼血便消失有效率为84.4%,对照组有效率为48.4%,两组在功能性便秘引发肛裂伴血便早期治疗中,治疗组优于对照组(P<0.05)。两组FC儿童在治疗2周后,治疗组便秘总有效率为96.9%,对照组便秘总有效率为93.5%,两组便秘疗效相当(P>0.05);治疗组肛裂伴肉眼血便消失有效率为100%,对照组有效率为96.8%,两组在功能性便秘引发肛裂伴血便治疗中疗效相当(P>0.05)。在治疗停药后3个月时,治疗组便秘、肛裂伴血便的复发率均低于对照组(P<0.05)。结论九华痔疮栓联合乳果糖口服液、培菲康治疗小儿功能性便秘早期引发肛裂伴血便的治疗疗效远远优于口服乳果糖口服液联合培菲康,极大地降低了儿童便秘早期的排便恐惧,同时可加速修复肠道黏膜,并降低远期肛裂伴血便的复发。

Objective To explore the clinical efficacy of Jiuhua hemorrhoids suppository plus lactulose solution and bifico in the treatment of children with functional constipation and hematochezia caused by anal fissure. Methods A total of 63 children with functional constipation combined with hematochezia, who were admitted to hospital from March 2017 to March 2019, selected and were randomly divided into two groups. Thirty-two patients in the experimental group, were treated with Jiuhua hemorrhoids suppository plus lactulose solution and bifico. Thirty-one patients in the control group, were treated with lactulose solution and bifico. The two groups were all asked to improve bad eating habits and defecation training, and the course of treatment was 2 weeks. Results After 3 days of treatment, the total efficacy rate of constipation was 75.0% in the treatment group, which similar to 74.1% in the control group(P>0.05);The disappearance rate of anal fissure with hematochezia of the treatment group was 84.4%, which was better than 48.4% of the control group(P<0.05). After 2 weeks of treatment, the total efficacy rate of constipation was 96.9% in the treatment group, which similar to 93.5% in the control group(P>0.05);The disappearance rate of anal fissure with hematochezia of the treatment group was 100%, which was similar to 96.8% of the control group(P>0.05). After three-month discontinuation of treatment, the recurrence rate of constipation, anal fissure and hematochezia in the treatment group were lower than those in the control group(P<0.05). Conclusion The curative effect of Jiuhua hemorrhoids suppository plus lactulose solution and bifico is much better than that of lactulose solution and bifico in the treatment of children with functional constipation and hematochezia caused by anal fissure, which can greatly reduce children’s fear of defecation in early stage of constipation, also can accelerate the repair of intestinal mucosa and reduce the recurrence of long-term anal fissure and hematochezia.
