米非司酮周期疗法治疗围绝经期功能失调性子宫出血患者的效果

Effects of Mifepristone cycle therapy on patients with dysfunctional uterine bleeding during perimenopausal period

目的:观察米非司酮周期疗法治疗围绝经期功能失调性子宫出血(DUB)患者的效果。方法:选取84例围绝经期DUB患者为研究对象,按照随机数字表法分为对照组和观察组各42例。所有患者均给予诊断性刮宫及常规治疗,在此基础上对照组给予米非司酮持续治疗,观察组给予米非司酮周期疗法治疗。比较两组疗效、治疗前后性激素[促卵泡激素(FSH)、促黄体激素(LH)、孕酮(P)和雌二醇(E_(2))]水平、血红蛋白水平、子宫内膜厚度和不良反应发生率。结果:观察组治疗总有效率为97.62%,高于对照组的78.57%,差异有统计学意义(P<0.05);治疗后,两组LH、FSH、P、E_(2)水平和子宫内膜厚度均低于治疗前,且观察组低于对照组,两组血红蛋白水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:采用米非司酮周期疗法可提高围绝经期DUB患者治疗总有效率和血红蛋白水平,降低性激素水平和子宫内膜厚度,优于米非司酮持续治疗效果。

Objective:To observe effects of Mifepristone cycle therapy on patients with dysfunctional uterine bleeding(DUB)during perimenopausal period.Methods:84 DUB patients during perimenopausal period were selected as the research objects,and were divided into control group and observation group according to the random number table method,42 cases in each group.All patients were given diagnostic curettage and conventional treatment.On this basis,the control group was given continuous Mifepristone treatment,while the observation group was given Mifepristone cycle therapy.The treatment effects,the levels of sex hormones[follicle stimulating hormone(FSH),luteinizing hormone(LH),progesterone(P)and estradiol(E_(2))],the hemoglobin level,the endometrial thickness and the incidence of adverse reactions were compared between the two groups before and after the treatment.Results:The total effective rate of treatment in the observation group was 97.62%,which was higher than 78.57%in the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of LH,FSH,P,E_(2) and endometrial thickness in the two groups were lower than those before the treatment,and that of observation group was lower than that of the control group;and the hemoglobin levels were higher than those before the treatment;and that of observation group was higher than that of the control group;and the differences were statistically significant(P<0.05).However,there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Mifepristone cycle therapy can improve the total effective treatment rate and the hemoglobin level of the DUB patients during perimenopausal period,and reduce the levels of sex hormones and the endometrial thickness.Moreover,it is superior to continuous Mifepristone treatment.
