循序渐进式康复护理在行PVP治疗的老年胸腰椎压缩性骨折患者中的应用效果

Application effects of step by step rehabilitation nursing in elderly patients with thoracolumbar vertebral compression fractures undergoing PVP

目的:观察循序渐进式康复护理在行经皮穿刺椎体成形术(PVP)治疗的老年胸腰椎压缩性骨折患者中的应用效果。方法:选取89例行PVP治疗的老年胸腰椎压缩性骨折患者为研究对象,按照随机数字表法分为研究组(n=45)和对照组(n=44)。对照组采用常规护理,研究组在对照组基础上采用循序渐进式康复护理。比较两组护理前后视觉模拟评分法(VAS)评分、腰椎日本骨科协会评估治疗(JOA)评分、Barthel指数(BI)和并发症发生率。结果:护理后,研究组VAS评分低于对照组,腰椎JOA评分和BI高于对照组,差异均有统计学意义(P<0.05);研究组并发症发生率为3.67%(3/45),低于对照组的22.73%(10/44),差异有统计学意义(P<0.05)。结论:循序渐进式康复护理应用于行PVP治疗的老年胸腰椎压缩性骨折患者,可降低疼痛评分和并发症发生率,提高腰椎功能和日常生活活动能力,效果优于常规护理。

Objective:To observe application effects of step by step rehabilitation nursing in elderly patients with thoracolumbar vertebral compression fractures undergoing percutaneous vertebroplasty(PVP).Methods:A prospective study was conducted on 89 elderly patients with thoracolumbar vertebral compression fractures undergoing PVP.According to the random number table method,they were divided into study group(n=45)and control group(n=44).The control group used routine nursing,while the study group was given the step by step rehabilitation nursing on the basis of that of the control group.The visual analogue scale(VAS)score,the lumbar spine evaluation and treatment(JOA)score,the Barthel index(BI)and the incidence of complications were compared between the two groups before and after the nursing.Results:After the nursing,the VAS score of the study group was lower than that of the control group;the JOA score and BI of the lumbar spine were higher than those of the control group;and the differences were statistically significant(P<0.05).The complication rate in the study group was 3.67%(3/45),which was lower than 22.73%(10/44)in the control group,and the difference was statistically significant(P<0.05).Conclusions:On the basis of the routine nursing,the application of step by step rehabilitation nursing care can reduce the VAS score and the complication rate of the elderly patients with thoracolumbar vertebral compression fractures undergoing PVP,as well as increase the JOA score and BI.Moreover,it is superior to the routine nursing.
