重庆市渝中区MSW性行为特征及HIV、梅毒感染现状研究

Study on sexual behavior characteristics of MSW and HIV and syphilis infection status in Yuzhong District,Chongqing

目的了解重庆市渝中区男男性工作者(MSW)的人类免疫缺陷病毒(HIV)/梅毒感染情况及性行为特征,为MSW获得性免疫缺乏综合征(AIDS)、性病的健康干预提供科学依据。方法采用滚雪球法,招募148例MSW,完成调查问卷并进行HIV和梅毒检测。结果调查对象的HIV筛查阳性率为10.14%(15/148),梅毒筛查阳性率为8.11%(12/148)。单因素分析显示,HIV阳性与月收入、近6个月肛交安全套使用频率有关。多因素分析显示,月收入较高[比值比(OR)=0.219,95%置信区间(95%CI):0.056~0.852]、近6个月与男性发生肛交时每次使用安全套(OR=0.171,95%CI:0.044~0.658)是HIV感染的保护因素。单因素分析显示,梅毒阳性与月收入、近6个月与男性发生口交时安全套使用频次、AIDS防治知识知晓情况有关。多因素分析显示,近6个月与男性发生口交时每次使用安全套是梅毒感染的危险因素(OR=21.556,95%CI:2.571~180.724),知晓AIDS防治知识是梅毒感染的保护因素(OR=0.106,95%CI:0.026~0.441)。结论重庆市渝中区MSW的HIV和梅毒感染率较高,安全套使用率与AIDS知识知晓率偏低可能造成AIDS、性病的流行,需要进行重点干预。

Objective To understand the HIV/syphilis infection and sexual behavior characteristics of MSW in Yuzhong District,Chongqing,provide scientific basis for MSW's AIDS and STD health intervention.Methods The snowball method was used to recruit 148 MSW,complete the questionnaire and conduct HIV and syphilis testing.Results The positive rate of HIV screening among survey subjects was 10.14%(15/148),and the positive rate of syphilis screening was 8.11%(12/148).Univariate analysis showed that HIV positive was related to monthly income and the frequency of condom use during anal sex in the past 6 months.Multivariate analysis showed that high monthly income(OR=0.219,95%CI:0.056-0.852)and every use of condoms during anal sex with men in the past 6 months(OR=0.171,95%CI:0.044-0.658)were protective factors for HIV infection.Univariate analysis showed that positive syphilis was related to monthly income,frequency of condom use during oral sex with men in the past 6 months,and awareness of AIDS prevention knowledge.Multivariate analysis showed that every use of condoms during oral sex with men in the past 6 months(OR=21.556,95%CI:2.571-180.724)was a risk factor for syphilis infection,and knowledge of AIDS prevention and treatment(OR=0.106,95%CI:0.026-0.441)is a protective factor for syphilis infection.Conclusion The HIV and syphilis infection rate of MSW in Yuzhong District of Chongqing City is relatively high.The low condom use rate and AIDS knowledge awareness rate may cause the epidemic of AIDS and sexually transmitted diseases,and key interventions are needed.
