摘要 肺炎克雷伯菌(Klebsiella pneumoniae,KP)是一种革兰阴性杆菌,是导致医院感染的常见病原菌之一。随着抗菌药物的广泛使用,肺炎克雷伯菌对多种抗菌药物的耐药水平呈上升趋势,其中耐碳青霉烯肺炎克雷伯菌(carbapenem-resistant Klebsiella pneumoniae,CRKP)的出现给临床抗感染治疗带来了严峻挑战,研究肺炎克雷伯菌blaKPC耐药基因的传播机制对于临床防控具有重要意义。CRKP对碳青霉烯类抗菌药物产生耐药的机制是产生碳青霉烯酶,而KPC型碳青霉烯酶是肺炎克雷伯菌产生的最主要的一种碳青霉烯酶。blaKPC基因的传播可以通过不同的分子机制介导,本文主要就blaKPC耐药基因在肺炎克雷伯菌中的传播机制做一综述。 Klebsiella Pneumoniae,a gram-negative bacterium,is one of the common pathogens causing hospital-acquired infections.With the widespread use of antibacterial drug,the resistant rates of Klebsiella pneumoniae to various antibiotics is on the rise,among of which the emergence of carbapenem-resistant Klebsiella pneumoniae(CRKP)poses a serious challenges to clinical anti-infection treatment.It has great significance for clinical prevention and control to study the transmission mechanism of blaKPC resistance gene of Klebsiella pneumoniae.The mechanisms of CRKP's resistance to antibacterial drugs is due to the production of carbapenemase.KPC-type carbapenemase is the most common carbapenemase in Klebsiella Pneumoniae.The spread of the blaKPC gene can be mediated by different molecular mechanisms.This article mainly reviews the transmission mechanism of blaKPC resistance gene in Klebsiella pneumoniae.
