鹤膝膏联合双氯芬酸钠缓释片治疗膝骨关节炎临床研究

Clinical Study of Hexi Ointment Combined with Diclofenac Sodium Sustained-Release Tablets in the Treatment of Knee Osteoarthritis

目的探讨鹤膝膏联合双氯芬酸钠缓释片治疗膝骨关节炎的疗效,以及对患者膝关节活动度的影响。方法选取医院2019年5月至2020年1月收治的膝骨关节炎患者90例,随机分为观察组和对照组,各45例。两组患者均予双氯芬酸钠缓释片治疗,观察组患者加用鹤膝膏,均以4周为1个疗程,连续治疗3个疗程。结果观察组的总有效率为88.89%,显著高于对照组的71.11%(P <0.05)。与对照组比较,观察组患者治疗后的中医证候(包括肿胀、疼痛、僵硬、跛行)积分显著更低(P <0.05);膝关节活动度(包括屈曲度、伸直度、屈伸弧度)显著更高(P <0.05);炎性因子包括基质金属蛋白酶3(MMP-3)、白细胞介素8(IL-8)、肿瘤坏死因子-α(TNF-α)水平均显著更低(P <0.05)。治疗期间,观察组和对照组不良反应发生率相当(13.33%比8.89%, P> 0.05)。结论鹤膝膏联合双氯芬酸钠缓释片治疗膝骨关节炎疗效较好,可改善患者的中医证候积分,提高膝关节活动度,降低机体炎性因子水平,且治疗安全性良好。

Objective To investigate the efficacy of Hexi Ointment combined with Diclofenac Sodium Sustained-Release Tablets in the treatment of knee osteoarthritis and its effect on the knee range of motion.Methods A total of 90 patients with knee osteoarthritis admitted to our hospital from May 2019 to January 2020 were selected and randomly divided into the observation group and the control group,45 cases in each group.The patients in the two groups were treated with Diclofenac Sodium Sustained-Release Tablets,on this basis,the patients in the observation group were treated with Hexi Ointment.Both groups were treated for three consecutive courses with four weeks as a course of treatment.Results The total effective rate of the observation group was 88.89%,which was significantly higher than 71.11%of the control group(P<0.05).After treatment,the scores of TCM syndromes including swelling,pain,stiffness and claudication in the observation group were significantly lower than those in the control group(P<0.05).The knee range of motion including flexion,extension and flexion and extension radian in the observation group was significantly higher than that in the control group(P<0.05).The levels of inflammatory factors including matrix metalloproteinase-3(MMP-3),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)in the observation group were significantly lower than those in the control group(P<0.05).During the treatment,the incidence of adverse reactions in the observation group was similar to that in the control group(13.33%vs.8.89%,P>0.05).Conclusion Hexi Ointment combined with Diclofenac Sodium Sustained-Release Tablets is effective and safe in the treatment of knee osteoarthritis,which can improve the scores of TCM syndromes,improve the knee range of motion,and reduce the level of inflammatory factors.
