^(18)FFDG PET/CT代谢参数SUL_(max)在肺癌中的应用价值分析 被引量:1

摘要 目的:分析18F-FDG PET/CT代谢参数最大标准摄取值(SUV_(max))、最大去脂肪标准摄取值(SUL_(max))、代谢体积(MTV)、病灶糖酵解总量(TLG)与肺癌患者临床病理因素的关系,探讨SUL_(max)较其他参数是否有更高临床应用价值。方法:选取行18F-FDG PET/CT检查并经手术病理证实为肺癌的患者102例。按照不同临床病理资料进行分组。呈正态分布的资料采用独立样本t检验或单因素方差检验对各组间代谢参数进行比较,采用Pearson相关性分析观察各代谢参数与原发灶最大径及各临床病理因素的相关性;非正态分布的数据,组间比较采用Mann-Whitney U非参数检验或Kruskal-Wallis秩和检验,采用Spearman相关性分析观察各代谢参数与临床病理因素的相关性。以P<0.05为差异有统计学意义。结果:肿瘤原发灶大小、患者性别、T分期、N分期、组织病理类型组间的代谢参数SUV_(max)、SUL_(max)、MTV、TLG差异有统计学意义。各代谢参数与肿瘤原发灶最大径、T分期呈正相关,与N分期呈较弱正相关。结论:18F-FDG PET/CT代谢参数SUV_(max)、SUL_(max)、MTV、TLG与肺癌患者各临床病理参数具有一定的相关性;SUL_(max)与患者各临床病理参数的关系与SUV_(max)大致相仿,而MTV、TLG与前两者相比,可能具有更高的临床应用价值。 Purpose:To analyze the relationship between ^(18)F-FDG PET/CT metabolic parametersmaximum standardized uptake value(SUV_(max)),maximum standardized uptake value normalized by lean body mass(SUL_(max)),metabolic tumor volume(MTV),total lesion glycolysis(TLG)and various clinicopathological factors in lung cancer patients.And to explore whether SUL_(max)has higher clinical application value than the other parameters.Methods:A total of 102 patients with pathologically confirmed lung cancer were enrolled and their PET/CT examination results were selected.The patients were grouped according to the different clinicopathological types.The measurement data with normal distribution were compared by independent sample t-test or univariate variance test.The correlation between metabolic parameters and the maximum diameter of primary foci and various clinicopathological factors was observed by Pearson correlation analysis.For non-normal distribution data,Mann-Whitney U non-parametric test or Kruskal-Wallis rank sum test were used for comparison between different groups,and Spearman correlation analysis was used for observation of the correlation between metabolic parameters and clinicopathological factors.Result:There were statistically significant differences in the metabolic parameters of SUV_(max),SUL_(max),MTV and TLG between the primary tumor size,the gender of patients,T stages and N stages.The metabolic parameters were positively correlated with the maximum diameter and T stage of the primary tumor,and were weakly positively correlated with N stage.Conclusion:^(18)F-FDG PET/CT metabolic parameters SUV_(max),SUL_(max),MTV and TLG were correlated with various clinicopathological factors in patients with lung cancer.The relationship between SUL_(max)and the clinicopathological factors of lung cancer patients is roughly similar to SUV_(max),MTV,TLG may have higher clinical application value than the above metabolic parameters.
