Olympus小探头超声术在老年非小细胞肺癌患者淋巴结分期中的诊断价值

Olympus small ultrasound probe for the diagnosis of lymph node staging in elderly patients with non-small cell lung cancer

目的探讨奥林巴斯(Olympus)小探头超声术在老年非小细胞肺癌患者淋巴结分期中的诊断价值。方法选取2018年2月至2020年9月间通用环球西安西航医院诊治的70例老年非小细胞肺癌患者作为研究对象,均采用Olympus小探头超声检查,记录影像学特征,判断淋巴结转移与分期情况。结果在70例患者中,超声判断为淋巴结转移的24例患者纳入转移组,其中Ⅰ期13例,Ⅱ期7例,Ⅲ期4例。转移组的超声低回声、边界清晰、回声不均、类圆形和短径≥10mm比例均高于非转移组,差异均有统计学意义(均P<0.05)。转移组的超声弹性分型与非转移组对比,差异有统计学意义(P<0.05)。病理判断为淋巴结转移26例,其中Ⅰ期14例,Ⅱ期8例,Ⅲ期4例,超声诊断淋巴结分期的准确性为94.3%。超声对淋巴结分期的诊断敏感性与特异性均>85.0%。结论Olympus小探头超声术应用于老年非小细胞肺癌患者有很好的影像学特征,对淋巴结分期的诊断具有较高的准确度、敏感性与特异性。

Objective To investigate the diagnostic value of Olympus small ultrasound probe for lymph node staging in elderly patients with non-small cell lung cancer. Methods From February 2018 to September 2020,70 elderly patients with non-small cell lung cancer were selected at General Global Xi’an Xihang Hospital. Olympus small ultrasound probe was performed on all patients,the imaging characteristics were recorded and lymph node metastasis and staging were assessed. Results Of the 70 patients,24 patients were diagnosed as lymph node metastasis by ultrasound and were included in a metastasis group including 13 patients with stage Ⅰ cancer,7 patients with stage Ⅱ cancer,and 4 patients with stage Ⅲ cancer.The proportion of hypoechoic ultrasound,clear boundary,uneven echo,round shape,and short diameter ≥10 mm were higher in the metastatic group than in the non-metastatic group( all P < 0. 05). Compared with the non-metastatic group,the ultrasound elastic classification in the metastatic group was significantly different( P < 0. 05). Pathologically showed that 26 patients had lymph node metastasis including 14 patients with stage Ⅰ cancer,8 patients with stage Ⅱ cancer,and 4 patients with stage Ⅲ cancer. The accuracy of ultrasound diagnosis of lymph node staging was 94. 3%. The diagnostic sensitivity and specificity of ultrasound for lymph node staging exceeded 85. 0%. Conclusion Olympus small ultrasound probe shows good imaging characteristics in elderly patients with non-small cell lung cancer with high accuracy,sensitivity and specificity for the diagnosis of lymph node staging.
