蛋白酶激活受体2与骨癌患者疼痛程度的相关性研究

Relationship between protease activated receptor 2 and pain in patients with bone cancer

目的探讨蛋白酶激活受体2(PAR2)与骨癌患者疼痛程度的相关性。方法选取2017年10月至2020年10月间陕西中医药大学第二附属医院收治的80例骨癌患者,采用视觉模拟评分分为重度疼痛组35例、中度疼痛组26例和轻度疼痛组19例。比较三组患者的临床情况,分析PAR2与骨癌患者治疗前后疼痛程度的相关性。结果重度疼痛组患者PAR2阳性33例,中度疼痛组患者PAR2阳性19例,轻度疼痛组患者PAR2阳性12例,组间比较,差异有统计学意义(P<0.05)。镇痛治疗后,重度疼痛组11例,中度疼痛组31例,轻度疼痛组38例;三组患者性别、年龄、骨肿瘤外科分期、肿瘤位置、病理类型比较,差异无统计学意义(P>0.05);三组患者肺或全身骨转移和PAR2情况比较,差异均有统计学意义(均P<0.05)。Spearman相关分析显示,PAR2和肺或全身骨转移与镇痛治疗后骨癌患者的疼痛程度呈正相关(均P<0.05)。结论PAR2表达与骨癌患者的疼痛程度有一定相关性,通过阿片类药物止痛治疗后,PAR2会对止痛效果产生影响,为患者镇痛治疗提供参考。

Objective To investigate the correlation between protease activated receptor 2(PAR2)and pain in patients with bone cancer.Methods Eighty patients with bone cancer admitted to The Second Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine were selected from October 2017 to October 2020.The patients were categorized into a severe pain group(35 patients),a moderate pain group(26 patients)and a mild pain group(19 patients)by visual analogue scale(VAS).Clinical condition was compared among the three groups and the relationship between PAR2 and pain was analyzed before and after the treatment.Results Positive expression of PAR2 was observed in 33 patients in the severe pain group,19 patients in the moderate pain group and 12 patients in the mild pain group(all P<0.05).After analge-sic treatment,11 patients had severe pain,31 patients had moderate pain and 38 patients had mild pain.There was no significant differences in gender,age,surgical stage,tumor location and pathological type a-mong the three groups after analgesic treatment(P>0.05).However,there were significant differences in lung or systemic bone metastasis and the status of PAR2 among the three groups(all P<0.05).The results of Spearman correlation analysis showed that PAR2,lung or systemic bone metastasis were positively corre-lated with the degree of pain in patients with bone cancer after analgesic treatment(all P<0.05).Conclusion The expression of PAR2 is partly associated with the degree of pain in patients with bone cancer.After opioid analgesic treatment,the expression of PAR2 will influence the analgesic effect,which provides a reference for the analgesic treatment in patients with bone cancer.
