基于规范化管理的预防性护理对淋巴瘤住院化疗患者骨髓抑制期感染情况的影响

Impact of standardized management-based preventive care on infection in lymphoma inpatients undergoing chemotherapy during myelosuppression

目的探讨基于规范化管理的预防性护理对淋巴瘤住院化疗患者骨髓抑制期感染情况的影响。方法选取2019年7月至2020年7月间安康市中医医院收治的53例淋巴瘤住院化疗骨髓抑制期患者为观察组,2018年6月至2019年6月收治的53例淋巴瘤住院化疗骨髓抑制期患者为对照组。观察组患者采用基于规范化管理的预防性护理,对照组患者采用常规护理。比较两组患者干预4周期院内感染情况、干预前后自我管理能力、癌症自我管理效能感量表评分以及生活质量。结果干预4周期间,观察组感染发生率为7.5%,低于对照组的22.6%,差异无统计学意义(P<0.05)。干预4周后,两组患者的自我管理效能感和生活质量各项评分均比干预前升高,观察组患者自我管理效能感量表正性态度、缓解压力和自我决策评分均高于对照组,差异均有统计学意义(均P<0.05)。观察组患者生活质量量表中生理状况、社会家庭状况、情感状况及功能状况评分均高于对照组,差异均有统计学意义(均P<0.05)。结论基于规范化管理的预防性护理可有效预防淋巴瘤住院化疗患者骨髓抑制期感染情况,且有助于提高患者自我管理能力和生活质量。

Objective To explore the impact of preventive care based on standardized management on infection in patients with lymphoma on chemotherapy during the myelosuppressive phase.Methods Be-tween July 2019 and July 2020,53 patients with lymphoma who were admitted to Ankang Hospital of Tradi-tional Chinese Medicine and underwent myelosuppressive chemotherapy were included in an observation group.Between June 2018 and June 2019,53 patients with lymphoma who were admitted to Ankang Hospi-tal of Traditional Chinese Medicine and underwent myelosuppressive chemotherapy were included in a con-trol group.The observation group received preventive care based on standardized management and the con-trol group received conventional nursing care.The incidence of nosocomial infection was compared between the two groups over the intervention of 4 weeks.In addition,self-management ability was also compared be-tween the two groups including cancer self-management efficacy scale(C-SUPPH)Chinese version score and quality of life.Cancer quality-of-life measure was used to evaluate quality of life before the intervention and 4 weeks after the intervention.Results Over the intervention of 4 weeks,the incidence of infection was 7.5%in the observation group which was significantly lower than 22.6%of the control group(P<0.05).At 4 weeks after the intervention,the scores of self-management ability and quality of life were sig-nificantly increased in the two groups,and the scores of positive attitudes,stress remission and self-determination were significantly higher in the observation group than in the control group(all P<0.05).Moreo-ver,scores of physical performance,social and family status and emotional and function status were higher in the observation group than in the control group(all P<0.05).Conclusion Preventive care based on standardized management can effectively prevent nosocomial infection in patients with lymphoma during my-elosuppression on chemotherapy,and it can help to improve patients'self-management ability and quality of life.
