龙胆泻肝汤联合卡马西平治疗癫痫痰火内盛证临床研究

Clinical Study of Gentian Liver-Draining Decoction Combined with Carbamazepine in the Treatment of Phlegm Fire Excess Syndrome in Epilepsy

目的:观察龙胆泻肝汤联合卡马西平治疗癫痫痰火内盛证的临床疗效。方法:将80例癫痫患者按照随机数字表法分为观察组和对照组,每组各40例。对照组采用卡马西平治疗,观察组在对照组的基础上加用龙胆泻肝汤治疗。比较两组患者的临床疗效及治疗前后每周癫痫发作次数、癫痫发作持续时间、蒙特利尔认知评估量表(montreal cognitive assessment scale, MOCA)评分及中医证候积分变化情况。结果:观察组有效率为95.00%,对照组有效率为77.50%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后每周癫痫发作次数少于本组治疗前,且治疗后观察组少于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后癫痫发作持续时间短于本组治疗前,且治疗后观察组短于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后MOCA评分高于本组治疗前,且治疗后观察组高于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后中医证候积分低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。结论:龙胆泻肝汤联合卡马西平治疗癫痫痰火内盛证,可减少癫痫发作次数,缩短癫痫持续发作时间,改善患者的认知功能。

Objective:To observe the clinical curative effect of Gentian Liver-Draining Decoction combined with carbamazepine on phlegm fire excess syndrome in epilepsy.Methods:A total of 80 patients with epilepsy were randomly divided into the observation group and the control group,with 40 cases in each group.The control group was treated with carbamazepine,and the observation group was additionally treated with Gentian Liver-Draining Decoction on the basis of the control group.The clinical efficacy,weekly seizure frequency,seizure duration,Montreal Cognitive Assessment Scale(MOCA)score and TCM syndrome score of the two groups were compared between the two groups before and after treatment.Results:The effective rate was 95.00%in the observation group and 77.50%in the control group.There was significant difference between the two groups(P<0.05).The number of seizures per week after treatment in the two groups was fewer than that of the same group before treatment,and the number of seizures per week of the observation group was fewer than that in the control group after treatment,and the difference was statistically significant(P<0.05).The duration of epilepsy after treatment in the two groups was shorter than that of the same group before treatment,and the observation group′s was shorter than that in the control group after treatment,and the difference was statistically significant(P<0.05).The MOCA score of the two groups after treatment was higher than that of the same group before treatment,and the observation group′s after treatment was higher than that of the control group,and the difference was statistically significant(P<0.05).The TCM syndrome score of the two groups after treatment was lower than that of the same group before treatment,and the observation group′s after treatment was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Gentian Liver-Draining Decoction combined with carbamazepine in the treatment of phlegm fire excess syndrome in epilepsy can reduce the number of seizures,shorten the duration of seizures and improve the cognitive ability of patients.
