微创经皮肾镜取石术对输尿管上段结石患者清除效果及尿流动力学的影响

Influence of minimally invasive percutaneous nephrolithotomy on clearance effect and urodynamics in patients with upper ureteral calculi

目的探讨微创经皮肾镜取石术对输尿管上段结石患者清除效果及尿流动力学的影响。方法选取2018年1月至2019年12月本院收治的100例输尿管上段结石患者,依据随机数表法分为两组,各50例。对照组予以输尿管钬激光碎石手术,观察组予以微创经皮肾镜取石术。比较两组手术指标、结石清除效果以及尿流动力学改善情况。结果两组手术时间比较差异无统计学意义;与对照组比较,观察组术中出血量少于对照组,住院时间短于对照组,差异有统计学意义(P<0.05);观察组结石清除率为94.00%,高于对照组的76.00%,差异有统计学意义(P<0.05);观察组最大尿流率及最大尿道压力(MUP)均高于对照组,差异有统计学意义(P<0.05)。结论输尿管上段结石患者采用微创经皮肾镜取石术治疗效果显著,可有效减少术中出血量、缩短住院时间,提升结石清除率,改善尿流动力学指标,值得临床推广。

Objective To investigate the influence of minimally invasive percutaneous nephrolithotomy on clearance effect and urodynamics in patients with upper ureteral calculi.Methods 100 patients with upper ureteral calculi who admitted in the hospital from January 2018 to December 2019 were divided into two groups according to random number table,with 50 cases in each group.Control group underwent ureteral holmium laser lithotripsy,and observation group was treated with minimally invasive percutaneous nephrolithotomy.The surgical indexes,stone clearance effect and improvement of urodynamics were compared between the two groups.Results There was no statistical difference in the operation time between two groups,the intraoperative bleeding volume of the observation group was less than that of the control group,and the hospital stays of observation group was shorter than that in the control group,the difference was statistically significant(P<0.05).The stone clearance rate of observation group(94.00%)was higher than that 76.00%of control group,the difference was statistically significant(P<0.05).The maximum flow rate and maximum urethral pressure(MUP)in observation group were higher than those in control group,the difference was statistically significant(P<0.05).Conclusion Minimally invasive percutaneous nephrolithotomy in treatment of patients with upper ureteral calculi has significant curative effects,it can reduce intraoperative bleeding volume,shorten hospital stays,increase stone clearance rate,and imprvoe the indexes of urodynamics,which is worthy of clinical promotion.
