赣州市2006-2018年艾滋病抗病毒治疗的免疫学效果及影响因素分析

Analysis on immunological effect of AIDS HAART and its influencing factors in Ganzhou from 2006 to 2018

[目的]了解赣州市2006-2018年艾滋病抗病毒治疗的免疫学效果及其影响因素,为赣州市艾滋病抗病毒治疗工作提供科学依据。[方法]采用回顾性调查方法评估艾滋病抗病毒治疗的免疫学效果。在艾滋病防治基本信息系统——抗病毒治疗管理数据库中下载治疗病例的历史卡片,对治疗数据进行关联筛选和分析。[结果]调查的1520例艾滋病患者在坚持治疗后,CD4^(+)T淋巴细胞计数出现了明显上升,且治疗时间越长,CD4^(+)T淋巴细胞计数上升越多。1520例艾滋病患者中有326例患者发生了免疫学失败,失败率21.4%,50.0%发生在接受治疗后的16个月。多因素logistic回归分析显示年龄、文化程度为中学或中专、开始治疗年份为2016年及以后、服药依从性≤95%是2006-2018年赣州市艾滋病患者抗病毒治疗免疫学效果的影响因素。[结论]赣州市艾滋病抗病毒治疗免疫学失败率较高。赣州市应做到早发现、早治疗,并在对艾滋病患者开展抗病毒治疗的同时,加强服药依从性的教育,加快提升检测能力。

Objective To understand immunological effect of AIDS HAART and its influencing factors in Ganzhou from 2006 to 2018 so as to provide scientific basis for AIDS antiviral treatment in Ganzhou. Methods The retrospective survey method was used to evaluate the immunological effect of AIDS antiviral treatment. Downloaded history cards of treatment cases in AIDS prevention and treatment basic information system —— the antiviral treatment management database, and conducted relevant screening and analysis for treatment data. Results There were 1520 AIDS patients in this study after insisting treatment, the CD4^(+) T lymphocyte count increased significantly, and the longer the treatment time was the more CD4^(+) T lymphocyte count rose. There were 326 AIDS patients immunological failure among the 1520 patients of this survey, which failure rate was 21.4%,and 50% of the failure occurred after 16 months of receiving treatment. Multivariate logistic regression analysis showed that age, middle school or technical secondary school education, the treatment in 2016 and later, medication compliance ≤95% were the influencing factors of immunological effect of antiviral treatment for AIDS patients in Ganzhou from 2006 to 2018. Conclusions Immunological failure rate in Ganzhou is relatively high. It is suggested that should achieve early detection and early treatment, strengthen education on medication compliance and improve the testing capabilities while carting out antiviral treatment for AIDS patients in Ganzhou.
