清热解毒祛瘀方联合支气管镜下肺泡灌洗治疗支气管扩张急性加重期的临床研究

Clinical study of Qingre Jiedu Quyu formula combined with bronchoscopic alveolar lavage in the treatment of acute exacerbation of bronchiectasis

目的:探讨运用清热解毒祛瘀方联合支气管镜下肺泡灌洗治疗支气管扩张急性加重期患者的临床研究。方法:选取支气管扩张急性加重期患者104例为研究对象,随机分为观察组和对照组,每组52例,对照组患者常规治疗方法包括吸氧、抗感染、祛痰、止血、体位引流、支气管肺泡灌洗、补液等支持对症治疗,观察组患者在对照组治疗的基础上加服清热解毒祛瘀方治疗,比较两组患者的临床治疗效果。结果:治疗前,两组患者的动脉血气分析无明显差异,白细胞介素-4(Interleukin-4,IL-4)、白细胞介素-6(Interleukin-6,IL-6)、转化生长因子-β(Transforming Growth Factor-β,TGF-β),CD3^(+)、CD4^(+)和CD8^(+)百分比水平比较,差异有统计学意义(P>0.05);治疗后,两组患者的IL-4、IL-6、TGF-β水平均显著降低(P<0.05),观察组患者的IL-4、IL-6、TGF-β水平低于对照组(P<0.05)。两组患者CD3^(+)、CD4^(+)和CD8^(+)百分比水平均有增加,观察组患者CD3^(+)、CD4^(+)和CD8^(+)百分比升高水平高于对照组(P<0.05),观察组患者喘息、咳嗽、肺部湿啰音、咳痰等症状体征消失时间均显著短于对照组(P<0.05),观察组有效率高于对照组(P<0.01)。结论:清热解毒祛瘀方联合支气管镜下肺泡灌洗治疗支气管扩张急性加重期患者,可显著降低炎症因子水平,提高患者免疫功能,提高临床治疗有效率,并能明显提高临床治疗效果。

Objective:To investigate the clinical study of Qingre Jiedu Quyu formula(清热解毒祛瘀方)combined with bronchoscopic alveolar lavage in the treatment of patients with acute exacerbation of bronchiectasis.Methods:104 patients with acute exacerbation of bronchiectasis were enrolled in the study and were randomly divided into the observation group and the control group,with 52 cases in each group.The conventional treatment methods in the control group included oxygen inhalation,anti-infection,sputum,hemostasis,and positional drainage,bronchoscopic lavage,rehydration and other support for symptomatic treatment,the observation group patients were treated with Qingre Jiedu Quyu formula on the basis of the treatment of the control group,and the clinical treatment effects of the two groups were compared.Results:Before treatment,there was no significant difference in arterial blood gas analysis between the two groups.The percentages of IL-4,IL-6,TGF-β,CD3^(+),CD4^(+) and CD8^(+) were statistically significant(P>0.05).After treatment,the levels of IL-4,IL-6 and TGF-βin the two groups were significantly lower(P<0.05).The levels of IL-4,IL-6 and TGF-βin the observation group were lower than those in the control group.(P<0.05).The percentages of CD3^(+),CD4^(+) and CD8^(+) were increased in both groups.The elevated levels of CD3^(+),CD4^(+) and CD8^(+) in the observation group were higher than those in the control group(P<0.05).The time of disappearance of symptoms and signs such as wheezing,cough,pulmonary rales and expectoration in the observation group was significantly shorter than that in the control group(P<0.05).The effective rate of the observation group was higher than that of the control group(P<0.01).Conclusion:Qingre Jiedu Quyu formula combined with bronchoscopic alveolar lavage can significantly reduce the level of inflammatory factors,improve the immune function of patients,promote the efficiency of clinical treatment,and significantly enhance the clinical treatment effect in treating patients with acute exacerbation of bronchiectasis.
