钼靶摄影下新版BI-RADS在乳腺可疑形态钙化中的应用价值

摘要 目的:探讨新版乳腺影像报告与数据系统(breast imaging report and system,BI-RADS)在乳腺可疑形态钙化中的应用价值。方法:回顾性分析269例含有可疑形态钙化的女性患者钼靶片,通过最新版(第5版)BI-RADS对钙化灶的形态、分布进行标准化描述,并进行分类。将钙化形态和分布与病理结果对照分析。根据病理结果算出其阳性预测值(positive predictive value,PPV),BI-RADS分级与算出的PPV值作对比。结果:269例可疑形态钙化中,呈弥漫性分布的各种可疑形态钙化的PPV均为0%(P<0.05),即可以归为良性;段样分布的细线样/细分枝状钙化的恶性度最高,PPV为100%(P<0.05),BI-RADS为5类。结论:新版BI-RADS中的可疑形态钙化同样适用于国人,第5版BI-RADS中可疑形态钙化良恶性的判断及其进一步处理的建议对临床具有重要意义。 Objective:To explore the application value of the new breast Imaging Report and System(BI-RADS)in the treatment of breast suspicious morphology calcification.Methods:A retrospective study was conducted to analyze the molybdenum samples of 269 female patients with suspected morphological calcification.The morphology and distribution of calcification foci were standardized and classified by the latest edition(5th edition)BI-RADS.The morphology and distribution of calcification were compared with pathological results,and the positive predictive value(PPV)was calculated according to pathological results,and the BI-RADS grading was compared with PPV.Results:BI-RADS were divided into 5 categories.Among 269 cases of suspicious morphological calcifications,all suspicious morphological calcifications with diffuse distribution could be classified as benign,with PPV of 0(P<0.05).The malignant degree of segmentalized fine-line/fine-branched calcification was the highest,with a PPV of 100%(P<0.05).Conclusion:Suspicious calcification in the new BI-RADS is suitable for Chinese people,and the judgment of benign and malignant suspicious calcification in the fifth BI-RADS and suggestions for further treatment are of great clinical significance.
