血清ICAM-1、IL-33水平对急性ST段抬高心肌梗死患者PCI治疗预后的影响 被引量:2

Influence of levels of ICAM-1 and IL-33 in serum on the prognosis of PCI in patients with acute ST segment elevation myocardial infarction

摘要 目的观察血清细胞间黏附分子-1(ICAM-1)、白细胞介素-33(IL-33)水平对急性ST段抬高心肌梗死(STEMI)患者经皮冠状动脉介入术(PCI)治疗预后的影响。方法抽取2017年3月至2019年3月黄河中心医院收治的97例STEMI患者为研究对象,患者均接受急诊PCI治疗,且治疗后获得1年随访结果,随访截至2021年3月;以患者PCI治疗后1年是否发生主要心血管不良事件(心力衰竭、全因死亡、非致死性心肌梗死、非计划再次血运重建等)作为预后观察指标并分组,分为预后良好组(70例)和预后不良组(27例)。所有患者均于入院时检测血清ICAM-1、IL-33水平,分析二者对STEMI患者行PCI治疗预后的影响。结果预后不良组患者入院时血清ICAM-1、IL-33水平均高于预后良好组(P<0.05);绘制受试者工作特征曲线(ROC曲线),入院时血清ICAM-1、IL-33单独及联合预测STEMI患者PCI治疗预后不良风险的曲线下面积(AUC)>0.80,均有一定预测价值;相关性检验发现,STEMI患者血清ICAM-1、IL-33水平呈正相关(r=0.268,P=0.008)。结论血清ICAM-1、IL-33在STEMI患者中过表达对STEMI患者PCI治疗预后具有一定影响,可能会增加预后不良风险。 Objective To observe the the influence of levels of intercellular adhesion molecule-1(ICAM-1)and interleukin-33(IL-33)on the prognosis of percutaneous coronary intervention(PCI)in patients with acute ST segment elevation myocardial infarction(STEMI).Methods Ninety-seven patients with STEMI admitted to Huanghe Central Hospital from March 2017 to March 2019 were selected as the study subjects,all patients were treated by PCI,and the results of 1 year of follow up was gained after treatment,the follow-up was up to March 2021;the prognosis of patients was evaluated and grouped as follows:whether the main adverse cardiovascular events(heart failure,all-cause death,non lethal myocardial infarction,unplanned revascularization)occurred in patients one year after PCI was taken and the prognostic indicators and divided them into two groups,the good prognosis groups(70 cases)and the poor prognosis groups(27 cases).The levels of serum ICAM-1 and IL-33 were measured at admission,the influence of the levels of serum ICAM-1 and IL-33 on the prognosis of PCI in patients with STEMI was analyzed.Results The levels of serum ICAM-1 and IL-33 of patients in the poor prognosis group were higher than those in the good prognosis group(P<0.05);the receiver operating characteristic(ROC)curve was drawn,the area under the curve(AUC)of serum ICAM-1 and IL-33 alone and in combination to predict the risk of poor prognosis after PCI in patients with STEMI was>0.80;the correlation test showed that the levels of ICAM-1 and IL-33 in patients with STEMI were positively correlated(r=0.268,P=0.008).Conclusions The overexpression of serum ICAM-1 and IL-33 has certain influence on the prognosis of PCI in patients with STEMI,which may increase the risk of adverse prognosis.
