摘要
目的探讨关节镜辅助微创手术治疗复杂性胫骨平台骨折的效果。方法抽取2017年1月至2020年10月长治市北大医疗潞安医院收治的复杂性胫骨平台骨折患者50例,按照手术方案将其分为两组。对照组24例行传统切开复位钢板内固定术,观察组26例行关节镜辅助微创手术。比较两组相关手术指标值,分析术后膝关节功能、疼痛程度、并发症情况。结果观察组膝关节功能优良率为92.31%(24/326),高于对照组的66.67%(16/24),χ2=5.128,P=0.024。观察组手术时间、下床活动时间、骨折愈合时间、视觉模拟评分表评分少于对照组(t=9.027、6.176、12.026、3.649,P<0.05),术后关节活动度、关节功能评分高于对照组(t=5.065、20.648,P<0.05)。两组术后并发症发生率比较,χ2=4.638,P<0.05。结论关节镜辅助微创手术可缩短复杂性胫骨平台骨折患者的手术时间和术后恢复时间,促进膝关节功能恢复,且术后并发症少。
Objective To investigate the therapeutic effect of arthroscopic-assisted minimally invasive surgery on complex tibial plateau fractures.Methods A total of 50 patients with complex tibial plateau fractures admitted to Changzhi Lu’an Hospital of Peking University Medical Center from January 2017 to October 2020 were divided into two groups according to the surgical plan.Twenty-four patients in the control group underwent traditional open reduction and steel plate internal fixation,and 26 patients in the observation group underwent arthroscopic-assisted minimally invasive surgery.Compared the relevant surgical index values between the two groups,and analyzed the postoperative knee joint function,pain degree,and complications.Results The excellent and good rate of knee joint function in the observation group was 92.31%(24/326),which was higher than 66.67%(16/24)in the control group,χ2=5.128,P=0.024.The operation time,time to get out of bed,fracture healing time,and visual analog scale scores in the observation group were lower than those in the control group(t=9.027,6.176,12.026,3.649,P<0.05),and the postoperative joint range of motion and joint function scores were higher than those in the control group(t=5.065,20.648,P<0.05).The incidence of postoperative complications between the two groups were compared,χ2=4.638,P<0.05.Conclusions Arthroscopic-assisted minimally invasive surgery can shorten the operation time and postoperative recovery time of patients with complex tibial plateau fractures,promote the recovery of knee joint function,and with few postoperative complications.
