超声弹性对比指数与常规超声鉴别甲状腺良恶性结节的价值 被引量:1

Value of elasticity contrast index and conventional ultrasound in differential diagnosis of benign and malignant thyroid nodules

摘要 目的研究超声弹性对比指数(ECI)与常规超声鉴别甲状腺良恶性结节的价值。方法抽取2020年1月至2020年12月于驻马店市中心医院诊治的94例甲状腺结节患者为研究对象,全部行常规超声检查及ECI评估,并以手术病理检查结果作为金标准,分析并比较ECI与常规超声鉴别甲状腺良恶性结节的临床价值。结果 94例甲状腺结节患者共107个甲状腺结节,经病理诊断良性结节49个,恶性结节58个;常规超声鉴别甲状腺良恶性结节的敏感性为63.27%,特异性为68.97%。甲状腺恶性结节的ECI值(3.51±1.12)高于甲状腺良性结节(2.59±0.85),P<0.05。绘制受试者工作特征曲线结果显示,ECI鉴别甲状腺良恶性结节的最佳界值为2.81,敏感性为89.66%,特异性为61.22%,曲线下面积为0.79,95%CI为0.705~0.866。结论超声弹性对比指数鉴别甲状腺良恶性结节的敏感性高于常规超声检查。 Objective To analyze the value of elasticity contrast index(ECI)and conventional ultrasound in the differential diagnosis of benign and malignant thyroid nodules.Methods Ninety-four patients with thyroid nodules diagnosed and treated in Zhumadian Central Hospital from January 2020 to December 2020 were selected as study subjects.All of them were subjected to conventional ultrasound examination and ECI assessment.Taking surgical and pathological results as the golden standard,the clinical value of ECI and conventional ultrasound in the differential diagnosis of benign and malignant thyroid nodules was analyzed and compared.Results Pathological results showed that there were 107 thyroid nodules in 94 patients,including 49 cases benign nodules and 58 cases malignant nodules,according to pathological diagnosis.The sensitivity and specificity of conventional ultrasound in the differential diagnosis of benign and malignant thyroid nodules were 63.27%and 68.97%,respectively.The ECI value of malignant thyroid nodules(3.51±1.12)was higher than that of benign thyroid nodules(2.59±0.85),P<0.05.The receiver operating characteristic curve showed that the best cut-off value,sensitivity,and specificity of ECI for differential diagnosis of benign and malignant thyroid nodules were 2.81,89.66%and 61.22%,respectively,while the area under the curve and the 95%CI were 0.79 and 0.705-0.866,respectively.Conclusions The sensitivity of ultrasound elasticity contrast index in differentiating benign and malignant thyroid nodules is higher than that of conventional ultrasound.
