银屑病患者皮损T淋巴细胞亚群的变化

A Study on the expression of T-lymphocyte subpopulation in psoriatic patients

目的:探讨银屑病患者皮损T淋巴细胞亚群的变化情况。方法:选择2018年1月至2019年1月来自中国人民解放军九六九医院皮肤科治疗的60例斑块型寻常型银屑病患者作为观察对象,同时选择同期来院诊治的健康人群60例作为对照组。在局麻情况下,用外科手术法切取躯干或四肢4 mm×4 mm大小的银屑病患者皮损组织与非皮损组织,同时以同样方式选取健康患者组织,检测皮损部位、非皮损部位和正常皮肤组织的T淋巴细胞亚群CD4+、CD8+、CD25+的原位表达。结果:银屑病患者皮损组织表皮和真皮CD4+、CD8+、CD25+细胞的原位表达高于正常组和非皮损组织(P<0.05);而非皮损组织中表皮和真皮的CD4+、CD8+、CD25+细胞的原位表达高于正常组(P<0.05);进行期皮损组织中表皮和真皮的CD4+的表达高于静止期(P<0.05);进行期皮损组织中真皮组织的CD25+表达高于静止期(P<0.05);进行期皮损组织中表皮组织的CD8+表达低于静止期(P<0.05)。结论:银屑病患者的皮损可能与T淋巴细胞亚群CD4+、CD8+、CD25+的活化与高表达有关。

Objective:To investigate the change of T lymphocyte subsets in psoriatic lesions.Methods:60 psoriatic patients treated in the Dermatological Department of 969 Hospital of PLA l from January 2018 to January 2019 were selected as the observation group,and 60 healthy persons treated by the hospital in the same period were selected as the control group.Skin lesions and non-skin lesions with the size of 4mm×4mm in trunk or extremities were collected from patients with psoriasis through surgical resection under local anesthesia.At the same time,the in situ expression of T lymphocyte subsets CD4+,CD8+and CD25+in skin lesions,non-skin lesions and normal skin tissues was detected by selecting healthy patients'tissues in the same way.Results:The in situ expression of CD4+,CD8+and CD25+cells in epidermis and dermis of psoriatic patients was significantly higher than that of the normal and non-skin tissues(P<0.05);The in situ expression of CD4+,CD8+and CD25+cells in epidermis and dermis of non-skin tissues was significantly higher than that of the normal tissues(P<0.05);The expression of CD4+in epidermis and dermis of patients with psoriasis in the active stage was significantly higher than that in the stationary stage(p<0.05);The expression of CD25+in epidermis and dermis of patients with psoriasis in the active stage was higher than that in the stationary stage,and the expression of CD25+in dermal tissue was significantly higher than that in the stationary stage(P<0.05);The CD8+expression in epidermis and dermis of patients with psoriasis in the active stage was lower than that in the stationary stage(P<0.05).Conclusion:The skin lesions of psoriatic patients may be related to the activation and the high expression of of the CD4,CD8 and CD25 of T lymphocyte subsets.
