膀胱经姜疗联合小青龙汤治疗外寒内饮型肺胀急性发作期的临床研究

Clinical study on the treatment of acute attack stage of lung distension due to external cold and internal fluid retention with the combination of bladder therapy with ginger and Xiaoqinglong decoction

目的:观察小青龙汤联合膀胱经姜疗方案治疗外寒内饮型肺胀急性发作期的临床疗效。方法:选取我院收治的122例外寒内饮型急性发作期肺胀患者为研究对象,随机分为观察组(n=64)和对照组(n=58)。两组患者均给予基础治疗,对照组给予膀胱经姜疗,观察组给予膀胱经姜疗+小青龙汤,比较两组患者临床疗效及治疗前后中医症状积分、肺功能、血清炎性因子(SAA)、降钙素原(PCT)、白细胞介素-6(IL-6)和白细胞介素-8(IL-8)水平的差异。结果:治疗后,两组患者临床总有效率分别为90.62%和75.86%,差异具有统计学意义(P<0.05);两组患者咳嗽、鼻塞、喉中痰鸣和胸闷积分均较治疗前降低,观察组低于同一时间对照组(均P<0.05);两组患者PaO_(2)和SaO_(2)水平均较治疗前升高,PaCO_(2)水平均较治疗前降低,观察组变化幅度较对照组大(均P<0.05);两组患者FVC、FEV1、FEV1%及PEF水平均较治疗前明显升高,观察组高于同一时间对照组(均P<0.05);两组患者SAA、PCT、IL-6及IL-8水平均较治疗前降低,观察组低于同一时间对照组(均P<0.05)。结论:膀胱经姜疗联合小青龙汤治疗外寒内饮型肺胀急性发作期可有效调节患者炎症水平,减轻气流受限程度,改善肺功能及临床症状,因而联合方案应用价值较高。

Objective:To=observe the clinical effect of Xiaoqinglong decoction combined with bladder ginger therapy in the treatment of acute pulmonary distension of cold outside and drinking inside.Methods:A total of 122 patients with acute pulmonary distension of Hanneiyin type admitted to our hospital were selected as the research subjects and randomly divided into experimental group(n=64)and control group(n=58).Two groups of patients were given basic treatment,the control group given bladder by ginger therapy,treatment group given bladder by ginger+small qinglong decoction,compared two groups of patients before and after the clinical curative effect and treatment of traditional Chinese medicine symptom score,pulmonary function,serum inflammatory cytokines(SAA),calcitonin(PCT)and interleukin 6(IL 6)and interleukin 8(IL-8)level difference.Results:After treatment,the total clinical effective rate of 2 groups was 95.31%and 84.51%,respectively,the difference was statistically significant(P<0.05).The scores of cough,nasal obstruction,phlegm in the throat and chest tightness in 2 groups were lower than before treatment,and those in test group were lower than those in control group at the same time(all P<0.05).The levels of PaO_(2) and SaO_(2) in 2 groups were increased compared with before treatment,and the levels of PaCO_(2) in 2 groups were decreased compared with before treatment,and the change range of experimental group was greater than that of control group(all P<0.05).The levels of FVC,FEV1,FEV1%and PEF in 2 groups were significantly increased compared with before treatment,and those in experimental group were higher than those in control group at the same time(all P<0.05).The levels of SAA,PCT,IL-6 and IL-8 in 2 groups were decreased compared with before treatment,and those in test group were lower than those in control group at the same time(all P<0.05).Conclusion:Bladder transginger therapy combined with Xiaoqinglong decoction can effectively regulate the level of inflammation,reduce the degree of airflow limitation,and improve the lung function and clinical symptoms in the acute attack of lung distension with external cold and internal drinking.Therefore,the combined program has high application value.
