hs-CRP与清蛋白比值对冠状动脉慢血流预测价值

摘要 目的探讨超敏C反应蛋白(hs-CRP)与清蛋白(Alb)比值(CAR)对冠状动脉慢血流(CSF)的预测价值。方法选取2018年6月—2019年12月在青岛大学附属医院行冠状动脉造影(CAG)检查并确诊为CSF的病人83例(CSF组),以及同期行CAG检查并确诊为正常冠状动脉血流(NCF)的病人108例(NCF组)。比较两组间临床指标的差异,采用多元Logistic回归分析确定CSF的独立危险因素,采用受试者工作特征(ROC)曲线评估CAR对CSF的诊断价值。结果 CSF组的男性比例、hs-CRP水平以及CAR显著高于对照组(χ^(2)=5.609,t=-4.954、-5.252,P<0.05),Alb水平显著低于对照组(t=-3.878,P<0.05)。Logistic回归分析显示,性别(OR=1.356,P<0.05)及CAR(OR=1.543,P<0.05)是CSF的独立危险因素。ROC曲线分析显示,CAR的曲线下面积为0.715,最佳截断值为0.06,此时CAR预测CSF的灵敏度为85.5%、特异度为62.0%。结论 CAR是CSF的独立危险因素,对CSF有预测价值。 Objective To investigate the value of high-sensitivity C-reactive protein(hs-CRP)/albumin(Alb) ratio(CAR) in predicting coronary slow flow(CSF). Methods A total of 83 patients who underwent coronary angiography(CAG) and were diagnosed with CSF in The Affiliated Hospital of Qingdao University from June 2018 to December 2019 were enrolled as CSF group, and 108 patients who underwent CAG and were found to have normal coronary flow(NCF) during the same period of time were enrolled as NCF group. Clinical indices were compared between the two groups. The multivariate Logistic regression ana-lysis was used to identify the independent risk factors for CSF, and the receiver operating characteristic(ROC) curve was used to evaluate the value of CAR in the diagnosis of CSF. Results Compared with the NCF group, the CSF group had significantly hig-her proportion of male patients(χ^(2)=5.609,P<0.05), hs-CRP level(t=-4.954,P<0.05), and CAR(t=-5.252,P<0.05) and a significantly lower Alb level(t=-3.878,P<0.05). The Logistic regression analysis showed that sex(OR=1.356,P<0.05) and CAR(OR=1.543,P<0.05) were independent risk factors for CSF. The ROC curve analysis showed that CAR had an area under the ROC curve of 0.715 in predicting CSF, with a sensitivity of 85.5% and a specificity of 62.0% at the optimal cut-off value of 0.06. Conclusion CAR is an independent risk factor for CSF and has a certain value in predicting CSF.
