不同肩袖肌腱损伤动物模型建立与评价

ESTABLISHMENT OF ANIMAL MODELS OF DIFFERENT ROTATOR CUFF TENDON INJURIES AND THEIR EVALUATIONS

摘要 目的应用兔肩胛下肌肌腱建立肩袖肌腱不同损伤自然愈合模型,并进行生物力学、组织学评价。方法将18只4周龄兔随机分为部分组、新月组和L型组,分别建立左侧肩胛下肌部分损伤、新月型损伤和L型损伤模型,右侧实施假手术作为对照侧。术后4个月采用苏木精-伊红(HE)、Masson染色观察肌腱愈合情况,采用强力仪检测肌腱生物力学强度。结果兔肩胛下肌不同损伤模型建立成功,HE、Masson染色可见各组损伤部位被瘢痕组织填充。生物力学强度测试显示,部分组、L型组与新月组比较,实验侧/对照侧最大负荷百分比、实验侧/对照侧杨氏模量百分比差异有统计学意义(F=36.292、18.876,P<0.05)。结论不同肩袖损伤方式对肩袖自然愈合可产生不同影响,新月型损伤愈合较部分损伤及L型损伤更好。 Objective To establish natural healing models of different rotator cuff tendon injuries using the subscapularis tendons of rabbits, and to perform biomechanical and histological evaluations. Methods Eighteen 4-week-old rabbits were randomly divided into partial group, crescent group, and L-shaped group, in which partial injury, crescent injury, and L-shaped injury were made in the subscapularis muscle on the left side, while sham procedures were performed on the right side as controls. At 4 months after operation, hematoxylin-eosin(HE) staining and Masson staining were performed to observe the tendon healing;the biomechanical strength of the tendons was measured by a strength analyzer. Results Different subscapularis injury models were established successfully. HE and Masson staining showed scar tissue filling in the injured parts in each group. The biological strength testing showed that the partial group and the L-shaped group had significant differences in the percentages of maximum load and Young’s modulus on the experimental side/control side as compared with the crescent group(F=36.292,18.876;P<0.05). ConclusionDifferent types of rotator cuff injuries have different effects on the natural healing of rotator cuffs, and crescent injuries heal better than partial injuries and L-shaped injuries.
