美托洛尔治疗急性心力衰竭的效果及对患者心功能与血流动力学的影响

Effect of metoprolol in the treatment of acute heart failure and its influence on cardiac function and hemodynamics

目的观察美托洛尔治疗急性心力衰竭的效果及对患者心功能与血流动力学的影响。方法选取2019年3月-2020年6月福建省厦门市海沧医院收治的急性心力衰竭患者92例,依循双色球随机法分为试验组与对照组,每组46例。对照组采用常规治疗,试验组在对照组治疗的基础上给予美托洛尔治疗,比较2组临床疗效、治疗前后心功能指标及血流动力学指标。结果试验组总有效率为97.83%,高于对照组的80.43%(χ^(2)=7.180,P=0.007);治疗10 d后,2组左室舒张末期内径及左室收缩末期内径均低于治疗前,左室射血分数及每搏输出量均高于治疗前,且试验组降低/升高幅度大于对照组(P<0.01);治疗10 d后,2组中心静脉压、平均肺动脉压、肺循环阻力及体循环阻力均低于治疗前,且试验组低于对照组(P<0.01)。结论美托洛尔治疗急性心力衰竭的临床疗效较好,可改善患者的心功能与血流动力学,用药安全性较高,值得推广应用。

Objective To observe the effect of metoprolol in the treatment of acute heart failure and its effects on cardiac function and hemodynamics.Methods 92 patients with acute heart failure admitted to Xiamen Haicang Hospital,Fujian Province from March 2019 to June 2020 were selected,and they were randomly divided into experimental group and control group according to the double-color ball method,with 46 cases in each group.The control group was treated with conventional treatment,and the experimental group was treated with metoprolol on the basis of the treatment of the control group.The clinical efficacy,cardiac function indexes and hemodynamic indexes before and after treatment were compared between the two groups.Results The total effective rate of the experimental group was 97.83%,which was higher than 80.43%of the control group(χ^(2)=7.180,P=0.007);After 10 days of treatment,the left ventricular end diastolic and left ventricular end systolic diameters in the two groups were lower than before treatment,left ventricular ejection fraction and stroke volume were higher than before treatment,and the decrease/increase of the experimental group was greater than that of the control group(P<0.01);After 10 days of treatment,the central venous pressure,mean pulmonary artery pressure,pulmonary circulation resistance,and systemic resistance in the two groups were lower than before treatment,and the experimental group was lower than the control group(P<0.01).Conclusion Metoprolol has a good clinical effect in the treatment of acute heart failure,can improve the cardiac function and hemodynamics of patients,and has high drug safety,which is worthy of promotion and application.
