老年人认知衰弱影响因素的Meta分析

Meta analysis of the influencing factors of cognitive frailty in the elderly

目的:分析老年人发生认知衰弱(cognitive frailty,CF)的影响因素。方法:检索PubMed、Embase、Cochrane Library、Web of Science、中国知网、中国生物医学文献数据库、万方及维普数据库中有关老年人发生CF危险因素的研究,检索时限均从建库至2020年8月,对纳入文献进行质量评价,运用RevMan 5.3进行Meta分析。结果:共纳入15篇文献,CF组包含1475例,非CF组包含21719例。Meta分析结果显示,性别、年龄、饮酒史、抑郁、运动量、营养状况、居住方式、睡眠、骨关节炎、糖尿病史、婚姻状况比较差异均有统计学意义(均P<0.05),尚不能确定吸烟史与CF有关。结论:基于现有证据,年龄≥75岁、女性、抑郁、营养不良、睡眠障碍、独居、骨关节炎、糖尿病史、离异或丧偶是CF的危险因素,适量饮酒、规律运动是其保护因素。

Objective:To analyze the influencing factors of Cognitive Frailty(CF)in the elderly.Methods:Studies on risk factors of cognitive frailty in the elderly were retrieved from PubMed,Embase,Cochrane Library,Web of Science,CNKI,CBM,Wanfang and VIP databases.The search time was limited to the period from library construction to August 2020.The quality of the included literature was evaluated,and Meta analysis was performed using RevMan 5.3.Results:A total of 15 articles were included,including 1475 cases in the cognitive frailty group and 21719 cases in the non-cognitive frailty group.Meta analysis results showed that there were statistically significantdifferences in gender,age,history of alcohol consumption,depression,amount of exercise,nutritional status,living style,sleep,osteoarthritis,history of diabetes,and marital status(P<0.05),it was not clear that smoking history was associated with cognitive frailty.Conclusions:Based on available evidence,≥75 years of age,female,depression,malnutrition,sleep disturbance,living alone,osteoarthritis,diabetes history,divorced or widowed are risk factors for cognitive frailty in the elderly,while moderate alcohol consumption and regular exercise are protective factors.
